Description
The Hy antigen is one of eight members of the Dombrock blood group system. The Hy antigen is only commonly expressed in people with African ancestry and never seen in caucasians. Because the supply for antisera to Dombrock antigens is very limited and have weak reactions with Dombrock antigens, compatibility testing is best done by single nucleotide polymorphism methods. The two antibodies anti-Doa and anti-Dob have caused acute and delayed hemolytic transfusion reactions. There is little clinical information about the other Dombrock antibodies other than that none have caused hemolytic disease of the newborn. [AABB Technical Manual, 18th edition, pp354-355]

Description
Immunoglobulins (Ig), or antibodies, are multifunctional components of the immune system that mediate interactions between antigens and various cellular mechanisms.[UpToDate: function-and-clinical-applications-of-immunoglobulins] Structurally, immunoglobulins consist of two of the same type of heavy chain and two of the same type of light chain per molecule. The heavy chains consist of five types, IgA, IgG, IgD, IgM and IgE, which correspond to the five major classes of immunoglobulins. IgG is the most common followed by IgA, IgM, IgD, then IgE. Two types of light chains (known as Bence-Jones proteins) exist: kappa and lambda, either of which can combine with any of the heavy chains. Light chains produced more abundantly compared to heavy chains and are rapidly excreted in the urine. The light chains are either bound with the heavy chains (i.e., intact immunoglobulin molecules) or are free (not bound) in serum and urine specimens. Immunoglobulin levels are measured to detect or monitor of monoclonal gammopathies (e.g. myeloma, lymphoma, macroglobulinemia) and immune deficiencies. In LOINC, when free or bound is not stated in the Component for light chains, then the test is measuring and/or reporting results combined and not distinguishing between the two. For example, "Immunoglobulin light chain.kappa" refers to both free and bound kappa light chains. Likewise, "IgG+immunoglobulin light chain.kappa" represents both IgG heavy chains and free and bound kappa light chains. However, "IgG.kappa" represents intact IgG molecules containing kappa light chains, and does NOT include free light chains.
