LP250851-5 Ustekinumab
Ustekinumab (UST) is a human immunoglobulin G1κ (IgG1κ) that binds sub-unit p40 of interleukin-12 (IL-12) and interleukin-23 (IL-23). IL-12 and IL-23 are parts of the Th1 and Th17 inflammatory pathways, respectively, and are important in the development of psoriasis. Psoriasis has pathogenic similarities to psoriatic arthritis (PsA) which is an inflammatory disorder that is often unresponsive to tumor necrosis factor (TNF) inhibitor therapy. Also, many patients with PsA who initially respond well to TNF inhibitors eventually stop responding. UST is not a TNF inhibitor and offers patients with PsA an alternative treatment which has been proven to be effective and safe for short-term use. The safety for long-term use of UST still needs further study. PMID: 24101949

Antibodies to ustekinumab (ATU) have been detected in 6.5% of patients who were receiving ustekinumab for13 months. The presence of ATU is associated with reduced levels of ustekinumab and treatment failure. The number of patients who develop ATU is similar to the number of patients who failed therapy with adalimumab regardless of whether those patients were positive or negative for antibodies to adalimumab.
