Human Granulocytic Ehrlichiosis (HGE) is an illness caused by a rickettsial bacteria transmitted by deer and dog ticks to humans. With HGE, also known as Human Granulocytic Anaplasmosis (HGA), neutrophils become infected by Anaplasma phagocytophilum and clusters of small bacteria may be seen within the neutrophils on blood smears during the terminal phases of the infection. Patients may exhibit fever, headache, malaise, myalgia, no skin rash, decreased WBC and platelet count and mild liver injury may also occur be present. Information from Emerg. Anaplasma phagocytophilum is a rickettsial bacterium (formerly known as Ehrlichia phagocytophilum or Ehrlichia equi) that infects white blood cells and causes human granulocytic ehrlichiosis (HGE), also called human granulocytic anaplasmosis (HGA). A. phagocytophilum is a pathogen transmitted to humans by the bite of a tick (i.e., American dog tick, deer tick, and Lone Star tick), and may pose a risk of being transmitted by blood transfusion. The first symptoms of HGE/HGA normally begin one to two weeks after a tick bite, but a negative test for antibodies during the first seven to 10 days of illness is not always accurate. Both immunoglobulin M (IgM) and immunoglobulin G (IgG) antibodies usually rise near the end of the first week of illness. IgM antibodies are less specific than IgG antibodies and more likely to result in a false positive, so a concurrent IgG titer should also be run. Repeat testing several weeks after the original test, including comparison of antibody titers between the two time periods, is recommended. For IgG, the first sample typically shows a low or negative result, and the second typically shows a significant (four-fold) increase in IgG antibody levels. Babesia microti is a protozoan parasite that infects the red blood cells (RBCs) of humans. It is the infectious agent that causes Babesiosis and is the most common species to infect humans in the United States. Babesia microti is difficult to distinguish from the Plasmodium falciparum parasite that causes malaria, both on a stained blood smear and by the symptoms of infection. The initial diagnosis is most often the identification of the parasite in RBCs on a Geimsa-stained thin blood smear. Follow-up indirect fluorescent antibody (IFA) testing is 88-96% specific for Babesia microti infection. Symptoms of infection are noticeable 1-8 weeks after the tick bite. Infection in 25% of adults and 50% of children by Babesiosis microti is asymptomatic. The less severe symptoms of infection include hemolytic anemia, jaundice, shortness of breath, hemoglobinuria, which occur during parasitic multiplication. Patients without spleens experience a much higher rate of RBC infection (85%) versus an RBC infection rate of 1-10% in patients with spleens. Splenectomized patients are more likely to be symptomatic when infected with Babesia microti and often die within 5-8 days. The vector of Babesia microti is the Ixodidae tick and the reservoirs are believed to be the white-footed mouse (Peromuscus Leucopus Rafinesque), the microtus vole (Microtus spp.) and the white-tailed deer (Odocoileus virginianus).[https://web.stanford.edu/group/parasites/ParaSites2009/NaikhobaManobi_Babesia/NaikhobaManobi_Babesia.htm]

Borrelia burgdorferi is a Gram negative spirochete bacterium that causes Lyme disease. It is most commonly transmitted to humans by an infected tick. Typical symptoms include fever, headache, fatigue, and a characteristic skin rash called erythema migrans that usually spreads from the bite mark. If left untreated, infection can spread to joints, the heart, and the nervous system. Paralysis of one or both sides of the face may occur. Treatment often requires antibiotics. This bacterium is microaerophillic and slow-growing, explaining the long delays when diagnosing Lyme disease. There are a large number of sub-species which differ in clinical symptoms and presentation as well as geographic distribution. The life-cycle of B. burgdorferi is complex, requiring ticks, rodents, and deer as temporary hosts. Mice are the primary reservoir for the bacteria that are transmitted to deer by the tick Ixodes scapularis.
