Zinc transporter 8 (ZnT8) is an islet beta-cell secretory membrane protein. ZnT8 autoantibodies (ZnT8A) are present in type 1 diabetes mellitus (DM) and are a marker for disease. When combined with established autoantibody markers, including glutamate decarboxylase (GADA), protein tyrosine phosphatase IA2 (IA2A), antibodies to insulin (IAA), and islet cytoplasmic autoantibodies (ICA), the ZnT8 antibody test improves diagnostic sensitivity in suspected cases of type 1 DM. Islet cell antigen (ICA) 512 is a receptor-tyrosine phosphatase-like protein associated with the secretory granules of neuroendocrine cells. ICA-512 is also known as protein tyrosine phosphatase-IA2 (PTP-IA2) or tyrosine phosphatase (IA2). It is an autoantigen of type 1 diabetes. This is NOT the same as pancreatic islet cell. Studies on type-1 diabetic patient's relatives found that the development of one or more islet autoantibodies (including IA-2 antibody) was an early indicator of the relatives' progression to type-1 diabetes. Autoantibody profiles identifying patients destined to develop type-1 diabetes are usually detectable before age three. One study found that for relatives seropositive for IA-2 antibody, the likelihood of developing type-1 diabetes mellitus within five-years was 65.3%. Some type-1 diabetic patients are originally diagnosed as having type-2 diabetes due to symptom onset in adulthood, societal obesity, and initial insulin-independence. Detection of one or more islet auto-antibodies (including IA-2) may be used to distinguish these patients with "latent autoimmune diabetes" from type-2 diabetics.

Glutamate decarboxylase 65 (GAD65) antibody is useful to diagnose insulin dependent diabetes mellitus (IDDM, Type I diabetes), to assess the risk for developing IDDM, to predict the onset of IDDM and the risk of developing related endocrine disorders such as thyroiditis. Before clinical onset, Type I diabetes is characterized by lymphocytic infiltration of the islet cells and by circulating autoantibodies against a variety of islet cell antigens including GAD-65, IA-2 (a tyrosine phosphatase-like protein), and insulin (IAA). GAD has two isoforms, GAD65 and GAD67, which are highly homologous but found in distinct subsets of patients. PMID: 7532143 GAD65 antibodies are specific to patients with insulin-dependent diabetes. The presence of GAD antibodies in a patient recently diagnosed with diabetes indicates a high risk of requiring insulin within three years.
