Term Description

Alanine aminotransferase (ALT) is an enzyme found primarily in the cells of the liver and is used to monitor possible liver damage. The ALT can be elevated from a number of causes including viral Hepatitis A or B, severe liver damage resulting from toxins in the blood such as medication overdose, and extended collapse of the circulatory system. P-5-P, also known as vitamin B6, is a co-enzyme to ALT during amino group transfer. When the assay for ALT is run, it is recommended by the International Federation of Clinical Chemistry and Laboratory Medicine (IFCC) that P-5-P be added to the reaction to ascertain that all of the ALT enzyme activity is measured. If the patient is deficient in B6 and additional P-5-P were not added, a falsely low level of ALT enzyme activity could be reported. Alanine transaminase or ALT is an enzyme (EC 2.6.1.2) also called serum glutamic pyruvic transaminase (SGPT) or alanine aminotransferase (ALAT). It is found in serum and in various bodily tissues, but is most commonly associated with the liver. It catalyzes the two parts of the alanine cycle. It is commonly measured clinically as a part of a diagnostic evaluation of liver health. Significantly elevated levels of ALT often suggest the existence of other medical problems such as viral hepatitis, diabetes, congestive heart failure, bile duct problems, infectious mononucleosis, or myopathy. However, elevated levels of ALT do not automatically mean that medical problems exist. Fluctuation of ALT levels is normal over the course of the day, and ALT levels can also increase in response to strenuous physical exercise. The main function of ALT is to convert alanine into pyruvate. ALT is released into the bloodstream when cells containing it are damaged. Increased levels of ALT in the blood provide an early indicator of liver damage. ALT is often measured with aspartate aminotransferase (AST) as part of a liver panel. Pyridoxal phosphate is a derivative of Vitamin B6 and is used by enzymes in all organisms. The majority of enzymes which depend on pyridoxal phosphate are associated with pathways involving amino acids, such as amino group transfer, decarboxylation and other important biochemical processes. When measuring AST and/or ALT without pyridoxal-5-phosphate (P-5-P), the rate of the decrease of NADH is directly proportional to the AST/ALT (respectively)-activity in the specimen. Testing for AST and/or ALT with the addition of P-5-P stabilizes the transaminases in the specimen and avoids falsely low values in specimens of patients suffering from either myocardial infarction or liver disease, who have an insufficient amount of P-5-P in their blood.
