Infection with the hepatitis delta virus (hepatitis delta-agent, HDV) is always seen in association with hepatitis B Virus (HBV) infection, where it may occur as a simultaneous acute infection or as an acute super-infection superimposed upon a case of chronic HBV hepatitis. This is an RNA virus that requires the presence of HBV for viral replication to occur. The serologic diagnosis of HDV depends upon the finding of HDV antigen and/or the presence of anti-HDV. Patients will almost invariably have detectable hepatitis B surface antigen (HBsAg) and/or antibodies to hepatitis core antigen (anti-HBc) and hepatitis Be antigen (HBeAg). In patients with acute co-infections, hepatitis D antigen (HDVAg) appears early after HBsAg does and disappears with convalescence. Acute HDV infections are associated with anti-HDV IgM antibody and chronic cases usually only demonstrate IgG antibody. Both antibodies may eventually disappear following convalescence. The simultaneous assessment of anti-HBc IgM will help to differentiate co-infections (present) from super-infections (absent). Information is from ARUP Laboratories.

Hepatitis Delta Ab is detected in serum from patients who harbor the delta virus in the liver. It is a unique RNA virus that can cause either acute or chronic hepatitis. The virus is unique in that it requires HBV for replication -- therefore delta hepatitis develops only in HBS AG positive patients. The delta agent appears to circulate in the blood in low titer as a 35nm particle with a core of delta antigen and a surface component of HBsAG. The HBsAG encapsulates the delta agent and is, therefore, necessary for its replication. Infected individuals have delta viral particles in the liver and antibody to the virus. Infections with delta hepatitis may manifest itself in different cases: 1) as acute delta hepatitis concurrent with acute type B hepatitis; 2) as acute delta hepatitis occurring in a chronic HBsAG carrier; and 3) as chronic delta hepatitis occurring in a chronic HBsAG carrier. Acute delta hepatitis can be quite severe. Chronic delta hepatitis has been associated with cirrhosis and progressive chronic liver injury. Delta hepatitis should be suspected ONLY in patients who are HBsAG-positive. It is a common cause for acute hepatitis in the chronic HBsAG carrier especially if the patient is an intravenous drug abuser. The delta virus can cause a severe form of chronic hepatitis and should be suspected if there is sudden, unexplained deterioration or if acute liver disease is present in a HBsAG positive patient with HBsAG.
