Hepatitis A virus (Hep A or HAV) is an RNA virus that causes an acute infectious disease of the liver and is usually spread by the fecal-oral route. It is transmitted by ingestion of contaminated food or water or through direct contact with an infectious person. In developing countries, and in regions with poor hygiene standards, the incidence of infection with this virus is high. HAV infection produces a self-limited disease that usually does not result in chronic infection or chronic liver disease. Overall occurrence acute liver failure from Hepatitis A is rare (approx. 0.5%). The risk for symptomatic infection is directly related to age, with >80% of adults having symptoms compatible with acute viral hepatitis and the majority of children having either asymptomatic or unrecognized infection. Antibody produced in response to HAV infection persists for life and confers protection against reinfection. The disease can be prevented by vaccination, and hepatitis A vaccine has been proven effective in controlling outbreaks worldwide.
Although HAV is excreted in the feces towards the end of the incubation period, specific diagnosis is made by the detection of HAV-specific IgM antibodies in the blood. IgM antibody is only present in the blood following an acute hepatitis A infection. It is detectable from one to two weeks after the initial infection and persists for up to 14 weeks. The presence of IgG antibody in the blood means that the acute stage of the illness is past and the person is immune to further infection. IgG antibody to HAV is also found in the blood following vaccination and tests for immunity to the virus are based on the detection of this antibody.

Hepatitis A virus (HAV) is a small, non-enveloped single-stranded RNA virus that belongs to the Picornaviridae family, genus Hepatovirus. HAV has six genotypes (I to VI) in which genotypes I to III are human isolates and IV to VI are only simian isolates. Genotypes I to III are subdivided into groups A and B. Genotype I is the most frequently reported. PMID: 15914081 The clinical manifestations of HAV infection can vary significantly and range from asymptomatic infection, usually seen in young children, to fulminant hepatitis, which can result in death. HAV has a global distribution, and transmission primarily occurs by the fecal-oral route. The hepatitis B surface antigen (HBsAg) is most frequently used to screen for the presence of this infection. It is the first detectable viral antigen to appear during infection. However, early in an infection, this antigen may not be present and it may be undetectable later in the infection as it is being cleared by the host. The infectious virion contains an inner "core particle" enclosing viral genome. The icosahedral core particle is made of 180 or 240 copies of core protein, alternatively known as hepatitis B core antigen, or HBcAg. During this 'window' in which the host remains infected but is successfully clearing the virus, IgM antibodies to the hepatitis B core antigen (anti-HBc IgM) may be the only serological evidence of disease. Therefore most hepatitis B diagnostic panels contain HBsAg and total anti-HBc(both IgM and IgG).

All of the major laboratories whose web sites we explored perform a confirmatory test to verify positive results on their routine HBS Ag EIA test. Some indicate that the confirmatory test requires and an extra charge, some do not. We believe that at present (2011) the confirmatory test is usually a neutralization test. But only one lab that we reviewed specified the method as such. This term covers all confirmatory methods and will not require changing if/when confirmatory methods change.

Hepatitis C virus (HCV) is a small (50 nm in size), enveloped, single-stranded, positive sense RNA virus in the genus hepacivirus, which is in the Flaviviridae family. More than 170 million people worldwide are affected by hepatitis C, and the majority of patients with acute hepatitis C develop chronic HCV infection, which can result in liver cirrhosis, hepatic failure or hepatocellular carcinoma. The HCV core protein is a highly basic, RNA-binding protein that forms the viral capsid. The E1 and E2 envelope glycoproteins are necessary for viral entry and fusion. Non-structural proteins include NS2, NS3, NS4A and 4B, and NS5A and 5B. The NS2-NS3 complex functions as a protease, and NS3 also has a helicase/NTPase domain. NS4A is a cofactor of NS3 protease activity. NS3-NS4A protease is one target of anti-HCV drugs.
