17-hydroxyprogesterone (17-OHP) is a steroid that is a member of the adrenal biosynthetic pathway and measured in order to diagnose congenital adrenal hyperplasia (CAH). Classic CAH is suspected when virilisation is seen in females at birth. The diagnosis of CAH in boys can be delayed. CAH is characterized by decreased cortisol as a result of a mutation in the CYP21A2 gene, which encodes the enzyme 21-hydroxylase. Increased blood concentrations of 17-OHP indicate a deficiency in 21-hydroxylase since 17-OHP is the substrate for 21-hydroxylase. Reduced levels of 21-hydroxylase negatively affect the production of cortisol. CAH is among the most frequent genetic disorders in humans. The severity of the physical manifestations are related to the degree of 21-hydroxylase deficiency. PMID: 24711560
