Bile acids are steroid acids found predominantly in the bile of mammals. They are produced in the liver by the oxidation of cholesterol, and are stored in gallbladder and secreted into the intestine in the form of salts. They act as surfactants, emulsifying lipids and assisting with their absorption and digestion. Synthesis of bile acids is a major consumer of cholesterol. The body synthesizes about 800 mg of cholesterol per day and about half of that is used for bile acid synthesis. In total about 20-30 grams of bile acids are secreted into the intestine daily; about 90% of excreted bile acids are reabsorbed (by active transport in the ileum) and recycled. This is referred to as the enterohepatic circulation. Since bile acids are made from endogenous cholesterol, the enterohepatic circulation of bile acids may be disrupted as a way to lower cholesterol. This is the mechanism of action behind bile acid sequestrants.

The liver absorbs almost all bile except approximately 1% which spills to the peripheral circulation. With liver diseases, its ability to absorb bile acids may diminish, resulting in increased serum levels. Patients with intestinal malabsorption and metabolic disorders such as Gilbert's disease, Crigler-Najjar syndrome, or Dubin-Johnson syndrome do not exhibit elevated bile acids concentrations.

Alcoholic cirrhosis has been associated with extremely high levels of the taurine-conjugated BAs taurocholate, taurochenodeoxycholate, and tauroursodeoxycholate, moderately elevated levels of glycine-conjugated BAs, including glycocholate, glycochenodeoxycholate, and glycoursodeoxycholate, and mild elevation of nonconjugated BAs (cholate, chenodeoxycholate). Chenodeoxycholic Acid is one of two primary bile acids in humans, the other is Cholic Acid. Both of these bile acids are formed from cholesterol and converted by intestinal bacteria (through deconjugation and 7-alpha-hydroxylation) to the secondary bile acids, deoxycholic acid and lithocholic acid. Elevated concentrations often suggest impaired hepatic clearance due to liver disease. Serum Cholic Acid rises more in intrahepatic cholestasis of pregnancy than does Chenodeoxycholic Acid.

Deoxycholic Acid is a secondary bile acid. Bile acids are formed in the liver, from cholesterol, stored and concentrated in the gallbladder, and excreted into the intestines in response to food. Intestinal bacteria converts primary bile acids (Cholic Acid and Chenodeoxycholic Acid) through deconjugation and 7-alpha-hydroxylation, into secondary bile acids. Elevated concentrations often suggest impaired hepatic clearance due to liver disease. An increase in secondary bile acids as well as a rise in the ratio of LCA (Lithocholic acid) to DCA (Deoxycholic Acid) has been observed in gallstone sufferers.

Ursodiol, also known as ursodeoxycholic acid and the abbreviation UDCA, is one of the secondary bile acids, which are metabolic byproducts of intestinal bacteria. Primary bile acids are produced by the liver and stored in the gall bladder. When secreted into the colon, primary bile acids can be metabolized into secondary bile acids by intestinal bacteria. Primary and secondary bile acids help the body digest fats. Ursodeoxycholic acid helps regulate cholesterol by reducing the rate at which the intestine absorbs cholesterol molecules while breaking up micelles containing cholesterol. Because of this property, ursodeoxycholic acid is used to treat (cholesterol) gallstones non-surgically.
While some bile acids are known to be colon tumor promoters (eg. deoxycholic acid), others such as ursodeoxycholic acid are chemopreventive, perhaps by inducing cellular differentiation and/or cellular senescence in colon epithelial cells.

LP30845-9 Bile acid pattern
Bile acids (BAs) play a primary role in digesting dietary fats. Primary BAs are produced in the liver by either the classic or alternative pathway. CYP7A1 controls the production of primary BAs in the liver. The primary BAs (chenodeoxycholic acid and cholic acid) are conjugated to either glycine or taurine to create conjugated primary BAs, which are stored in the gallbladder. Once released into the intestinal circulation, primary BAs are modified by gut bacteria (e.g., deconjugated) to form secondary BAs. Secondary BAs are actively reabsorbed in the terminal ileum, and travel back to the liver through the portal circulation. PMID: 28249284
Different disorders are associated with various patterns of primary and secondary BA levels. Examples of disorders and their associated BA patterns include:
Disorders of BA synthesis (information from PMID: 18577977):
1) Inborn errors of bile acid synthesis including cerebrotendinous xanthomatosis, Zellweger syndrome, and a variety of other enzyme deficiencies - low or absent BAs
2) Bile acid conjugation defects - increased levels of unconjugated cholic and deoxycholic acid, lack of chenodeoxycholic acid

1) Inflammatory bowel disease - reduced deconjugation by gut bacteria, lower levels of secondary BAs
2) Irritable bowel syndrome - reduced deconjugation, increased levels of primary conjugated BAs, lower levels of secondary BAs

4) Obesity - lower levels of both primary BAs
