Healthy patients have the majority of light chains in serum bound to heavy chains. Low levels of free light chains (unbound) are normal due to the over-production and secretion of free light chains by plasma cells. In the kidneys, the free light chains are selectively reabsorbed by the tubular cells, so their presence in urine is likely due to secretion into the urinary tract. Elevation of monoclonal free light chains in serum or urine signal malignant plasma cell proliferation (multiple myeloma), primary amyloidosis, or light chain deposition disease. Elevation of polyclonal free light chains suggests autoimmune diseases such as systemic lupus erythematosus. (Information from Freelite Beckman Coulter IMMAGE II package insert, Version 23, Feb 2009)

Plasma or concentrated urine is deposited on a gel. With an electrical current the proteins are sorted in function of their size, after which antigens for each targeted type of immunoglobulin are deposited on the gel. Thus more or less narrow bands appear on the gel, marking the location of the various immunoglobulines.
Immunofixation tends to replace protein electrophoresis because:
• it is faster (results within three hours);
• it is somewhat more sensitive. Immunofixation may reveal an immunoglobulin missed out by protein electrophoresis, especially at low concentrations (less than 1 gram/litre);
• it can be partially automated and can be used in more laboratories;
• it is more easily read and interpreted.
Immunofixation is however only sensitive to immunoglobulins and is more expensive than protein electrophoresis.
