Porphyrins build up among patients with hereditary enzyme disorders known as porphyrias. Porphyrias affect the heme biosynthetic pathway and cause increased urine excretion of porphyrinogens, by-products known as porphyrins and porphyrinogen precursors-including delta aminolevulinic acid (ALA) and porphobilinogen (PBG). Coproporphyrin and uroporphyrin excretion in urine is markedly increased during acute attacks of hereditary coproporphyria. Increased uroporphyrinogen, uroporphyrin, and coproporphyrin excretion occurs in porphyria cutanea tarda. Increased coproporphyrin excretion alone lacks specificity and sensitivity for lead testing and is likely to occur secondary to other diseases. Protoporphyrins are tetrapyrroles containing the following side chains:methyl (4)propionic acid (2)vinyl (2)In the metabolism of porphyrin, protoporphyrin IX is created by the enzyme protoporphyrinogen oxidase, and the enzyme ferrochelatase converts it into heme. Protoporphyrins are deposited in the shells of the eggs of some birds as a brown or red pigment, either as a ground color or as spotting. This occurs in most passerine species, some ground-nesting non-passerines, such as waders, gulls, nightjars and sandgrouse, where it provides camouflage, and some parasitic cuckoos, which need to mimic their passerine hosts' eggs. Protoporphyrins strengthen the egg shell, and are deposited where the shell is too thin as a result of calcium shortage. Spotting therefore tend to be heavier where the local soil is calcium-deficient, and in the eggs laid last in a clutch.

A group of compounds containing the porphin structure, four pyrrole rings connected by methine bridges in a cyclic configuration to which a variety of side chains are attached. The nature of the side chain is indicated by a prefix, as uroporphyrin, hematoporphyrin, etc. The porphyrins, in combination with iron, form the heme component in biologically significant compounds such as hemoglobin and myoglobin.

Porphyrins are created as byproducts of porphyrinogens. Hereditary enzyme disorders leading to the buildup of porphyrins or porphyrinogens cause porphyrias and affect the heme biosynthetic pathway. Most of these disorders are inherited as autosomal dominant disorders. Increased urine porphyrins are seen with congenital erythropoietic porphyria, acute intermittent porphyria, hereditary coproporphyria, variegate porphyria, and porphyria cutanea tarda.
