Lymphocyte proliferation is used in the evaluation of patients with primary and secondary immunodeficiency diseases which affect T lymphocytes. Plant mitogens, including phytohemagglutinin [PHA] and pokeweed [PWM], are capable of inducing blastogenesis and stimulating lymphocyte proliferation. The panel is used to assess T cell function in patients having immunosuppressive therapy and to evaluate patients with cellular or humoral immunodeficiency diseases, which include SCID (Severe Combined Immunodeficiency), CVID (Common Variable Immunodeficiency), and DiGeorge syndrome. Lymphocyte proliferation to antigens and mitogens is performed by flow cytometry using a thymidine analogue called Edu®. Total lymphocytes are identified using a combination of the CD45 marker and FSC/SSC parameters, and total T cells with the CD3 marker from within the lymphocyte population. Three concentrations each for Candida (CA) and Tetanus (TT) (same for mitogens) are used and only the result at the concentration of antigen/mitogen that gives maximal response for that individual (since this can vary from person to person) is reported. The maximal % antigen-specific proliferating lymphocytes (CD45+) (numerator) relative to the total CD45+ lymphocyte population (denominator) is assessed. For T cells, the maximal response where the % CD3+ proliferating antigen-specific T cells is expressed relative to the total CD3+ T cell population is assessed.
