Aminoacyl-tRNA synthetases (ARS) are enzymes that link amino acids to tRNA molecules and therefore play a key role in protein synthesis. Each ARS is specific for a single amino acid. [NCBI Books: NBK22356] Autoantibodies to ARS are a hallmark of connective tissue diseases including anti-synthetase syndrome (ASS) and idiopathic inflammatory myopathies such as polymyositis (PM) and dermatomyositis (DM). ASS, PM and DM have overlapping features, including myositis, arthralgia, skin manifestations and interstitial lung disease. Antibodies to several specific ARS enzymes have been identified, including anti-histidyl (anti-Jo-1), anti-threonyl (anti-PL-7), anti-alanyl (anti-PL-12), anti-glycyl (anti-Ej), anti-isoleucyl (anti-Oj), anti-asparaginyl (anti-Ks), anti-phenylalanyl (anti-Zo), and anti-tyrosyl (anti-Ha). Different antibodies or patterns of antibodies have been associated with specific clinical features. Anti-Jo-1, anti-Ej, and anti-PL-7 antibodies are associated with myositis, including classic DM, clinically amyopathic DM (CADM) and PM. Anti-PL-12 alone is associated with CADM or interstitial lung disease (ILD). Anti-Ej and anti-PL-12 together are associated with CADM. Anti-Ks and anti-Oj are associated with ILD alone. PMID: 17392287 Identifying the particular anti-ARS antibodies associated with an individual patient's disease may help guide treatment and predict clinical outcomes.
