Amphetamine (AMPH) is a drug that stimulates both the central and peripheral nervous systems and is used to treat attention-deficit/hyperactivity disorder, narcolepsy and obesity. PMID: 17209801 AMPH increases extracellular monoamine neurotransmitters in the brain including dopamine, serotonin and norepinephrine. AMPH promotes presynaptic release of the neurotransmitters rather than blocking their re-uptake. Physiological responses to AMPH use include increased blood pressure, increased heart rate, increased body temperature, relaxed bronchial muscle and depressed appetite. Abuse of AMPH can cause powerful psychological dependence, tolerance, mild physical dependence associated with tachycardia, restlessness, irritability, insomnia, personality changes and an intense form of chronic intoxication psychosis similar to schizophrenia. Long-term users of AMPH can experience depression, impaired memory and motor skills most likely due to a drop in dopamine transporters and damage to dopaminergic and serotonergic neurons. Excretion of AMPH is in the urine and the rate of elimination is dependent on the pH of the urine. When GABA binds to GABA A, chloride channels open and allow the influx of chloride ions, which decrease the membrane potential and inhibit the neuron's action potential. They are used as sedatives, hypnotics, anesthetics, and anticonvulsants. Prolonged use can lead to physical tolerance and dependence, which has caused a significant decrease in their use over time. The benzodiazepines are a class of drugs with sedative, hypnotic, anxiolytic, anticonvulsant, amnestic and muscle relaxant properties. They are often used for short-term relief of severe, disabling anxiety or insomnia. Long-term use may lead to tolerance and dependency. They are believed to act on the GABA receptor GABAA, the activation of which dampens higher neuronal activity. They began to be widely prescribed for stress-related ailments in the 1960s and 1970s. Their chemical structure is based upon diazepine and phenyl groups. Common benzodiazepines include diazepam, oxazepam, chlorodiazepam, alprozolam, and temazepam.

The group of drugs called cannabinoids includes both the cannabinoid chemicals found in the cannabis plant as well as synthetic and pharmaceutical cannabinoids. The cannabinoids in cannabis include tetrahydrocannabinol (THC), cannabidiol (CBD), and cannabinol (CBN). THC is responsible for the psychoactive effects of cannabis and binds to the CB1 and CB2 cannabinoid receptors. CBD does not have psychoactive effects but rather has a variety of therapeutic properties including anticonvulsant, analgesic, anti-anxiety, and antiemetic. The THC to CBD ratio determines the physiologic effects of the drug. A high THC/CBD ratio results in euphoric and anxiolytic effects, while a low THC/CBD ratio results in sedative effects. Both natural and synthetic cannabinoid compounds are used for therapeutic purposes. Synthetic cannabinoids, like THC, act on the CB1 and CB2 receptors. Adverse reactions, including psychosis, confusion, agitation, tachycardia, myocardial infarction, seizures, and fatal overdoses, are more common with synthetic cannabinoids compared to cannabis. Cocaine is an alkaloid of Erythroxylon coca that is grown in the northern South American Andes, India, Africa and Java. It is used medicinally as a local anesthetic and vasoconstrictor in nasal surgery and to dilate pupils. Because cocaine is a powerful central nervous system stimulant it is among the most abused drugs. Cocaine abuse can lead to sudden death due to cardiotoxicity or the sequential development of hyperthermia, agitated delirium and respiratory arrest. Cocaine is metabolized in the liver to inactive benzoylecgonine (BZE). BZE is pharmacologically inactive and has a longer half-life than its parent drug cocaine. It is the substance most tested for when investigating suspected cocaine abuse. It is also used to treat depression, narcolepsy, brain injury, cancer, pain, cognitive disorders and to treat patients with human immunodeficiency virus infection. Methylphenidate can be admistered orally, intravenously or intranasally. This medication can be habit forming and has potential for abuse. Morphine is an extremely powerful opiate analgesic drug and is the principal active agent in opium. Like other opioids such as heroin, morphine acts directly on the central nervous system (CNS) to relieve pain, in particular by targeting the synapses of the arcuate nucleus. Side effects include impairment of mental performance, euphoria, drowsiness, lethargy, and blurred vision. It also decreases hunger, inhibits the cough reflex, and produces constipation. Morphine is highly addictive when compared to other substances, and tolerance and physical and psychological dependence develop quickly. Patients on morphine often report insomnia, visual hallucinations and nightmares.

Oxycodone is a potent and addictive opioid analgesic medication synthesized from thebaine. Its name is derived from codeine, which has a very similar chemical structure. It is effective orally and is marketed in combination with acetylsalicylic acid (Percodan, Endodan, Roxiprin) or paracetamol (Percocet, Endocet, Roxicet, Tylox) for the relief of pain. More recently, ibuprofen has been added to oxycodone (Combunox). Long term users may develop a tolerance or resistance to the drug. Abusers crush the tablets and injest the powder orally, intranasally, or via intravenous, intramuscular or subcutaneous injections.

It can be used to ease pain before, during and after an operation. It is often combined with acetaminophen in the preparation co-proxamol (Darvocet in the US and Capadex in AUS). It is an optical isomer of Levopropoxyphene. The racemic mixture is called Propoxyphene. Propoxyphene is now banned in many countries due to cardiac side effects, and has been removed from the US market since 2010.

Tricyclic antidepressants get their name from their three ringed molecular structure. The primary use of tricyclic antidepressants is to treat depression as well as neuralgic pain, chronic pain, and migraine prophylaxis. In children, tricyclic antidepressants are used to treat enuresis, obsessive-compulsive disorder, attention deficit hyperactivity disorder, school phobia, and separation anxiety. These drugs are are taken orally and are absorbed by the liver and then excreted through the kidneys. Tricyclic antidepressants block the absorption of neurotransmitters at nerve terminals, which makes the neurotransmitters more available in the brain. [MEDSCAPE_EMED:819204]
