Barbiturates are CNS depressants that primarily act on gamma-aminobutyric acid (GABA) and GABA A receptors. When GABA binds to GABA A, chloride channels open and allow the influx of chloride ions, which decrease the membrane potential and inhibit the neuron's action potential. They are used as sedatives, hypnotics, anesthetics, and anticonvulsants. Prolonged use can lead to physical tolerance and dependence, which has caused a significant decrease in their use over time. However, certain uses are still common, such as phenobarbital for refractory seizure disorders, butalbital for analgesia, and thiopental for anesthesia. PMID: 25824249

The benzodiazepines are a class of drugs with sedative, hypnotic, anxiolytic, anticonvulsant, amnestic and muscle relaxant properties. They are often used for short-term relief of severe, disabling anxiety or insomnia. Long-term use may lead to tolerance and dependency. They are believed to act on the GABA receptor GABAA, the activation of which dampens higher neuronal activity. They began to be widely prescribed for stress-related ailments in the 1960s and 1970s. Their chemical structure is based upon diazepine and phenyl groups. Common benzodiazepines include diazepam, oxazepam, chlorodiazepam, alprozolam, and temazepam.

The group of drugs called cannabinoids includes both the cannabinoid chemicals found in the cannabis plant as well as synthetic and pharmaceutical cannabinoids. The cannabinoids in cannabis include tetrahydrocannabinol (THC), cannabidiol (CBD), and cannabinol (CBN). THC is responsible for the psychoactive effects of cannabis and binds to the CB1 and CB2 cannabinoid receptors. CBD does not have psychoactive effects but rather has a variety of therapeutic properties including anticonvulsant, analgesic, anti-anxiety, and antiemetic. The THC to CBD ratio determines the physiologic effects of the drug. A high THC/CBD ratio results in euphoric and anxiolytic effects, while a low THC/CBD ratio results in sedative effects. Both natural and synthetic cannabinoid compounds are used for therapeutic purposes. Synthetic cannabinoids, like THC, act on the CB1 and CB2 receptors. Adverse reactions, including psychosis, confusion, agitation, tachycardia, myocardial infarction, seizures, and fatal overdoses, are more common with synthetic cannabinoids compared to cannabis. Methadone (also known as Symoron, Dolophine, Amidone, Methadose, Physeptone, Heptadon and many other names) is a synthetic opioid. It is used medically as an analgesic and a maintenance anti-addictive and reductive preparation for use by patients with opioid dependency. Because it is an acyclic analog of morphine, methadone acts on the same opioid receptors and thus has many of the same effects. Methadone is also used in managing severe chronic pain.

Opiates are narcotic drugs sometimes used to control pain, but have high abuse potential. Opiates tend to relax users. This group of drugs includes opium, morphine, and codeine. Synthetic opiates based on the phenanthrene nucleus of morphine include hydromorphone, hydrocodone, oxymorphone, oxycodone, dihydrocodeine, meperidine, and levorphanol. Opium comes in chunks and powder and may be eaten or smoked. Heroin is a manufactured opiate and is made into a powder that may be dissolved in water and injected. Oxycodone is a potent and addictive opioid analgesic medication synthesized from thebaine. Its name is derived from codeine, which has a very similar chemical structure. It is effective orally and is marketed in combination with acetylsalicylic acid (Percodan, Endodan, Roxiprin) or paracetamol (Percocet, Endocet, Roxicet, Tylox) for the relief of pain. More recently, ibuprofen has been added to oxycodone (Combunox). Long term users may develop a tolerance or resistance to the drug. Abusers crush the tablets and injest the powder orally, intranasally, or via intravenous, intramuscular or subcutaneous injections.

Phencyclidine (PCP) is a widely used hallucinogen that exhibits stimulating, depressing, and analgesic properties. PCP, also known as "angel dust", causes psychosis and may result in violent behavior. PCP may be a powder, tablet, liquid, spray or capsule, and it may be swallowed, smoked, sniffed, or injected. Effects include increased heart rate and blood pressure, flushing, sweating, dizziness, numbness, drowsiness, convulsions, and coma. PCP can be found 7 days following use and up to 2-4 weeks later in chronic users. Information from Jacobs and DeMott Laboratory Test Handbook, 5th edition, DS Jacobs, WR DeMott and DK Oxley, 2001.

Dextropropoxyphene is an analgesic in the opioid category structurally related to methadone. It is used to treat mild to moderate pain and as an antitussive. It can be used to ease pain before, during and after an operation. It is often combined with acetaminophen in the preparation co-proxamol (Darvocet in the US and Capadex in AUS). It is an optical isomer of Levopropoxyphene. The racemic mixture is called Propoxyphene. Propoxyphene is now banned in many countries due to cardiac side effects, and has been removed from the US market since 2010.
