Rouleaux (singular rouleau) are stacks of red blood cells (RBCs) which form because of the unique discoid shape of RBCs in vertebrates. The flat surface of the discoid RBCs provides a large surface area that allows the cells to stick to each other, thus forming rouleaux. Rouleaux occurs when the plasma protein concentration is high, leading to increased erythrocyte sedimentation rate. The presence of rouleaux does not always indicate the presence of a disease. Conditions which cause rouleaux formation include infections, inflammatory and connective tissue disorders, and cancers. It also occurs in diabetes mellitus and is one of the causes leading to microvascular occlusions in diabetic retinopathy.
