Beta aminoisobutyric acid (BAIB) is a nonprotein amino acid that is an end product of pyrimidine metabolism. The R and S enantiomers (previously named D-(minus)-BAIB and L-plus-BAIB) are produced from thymine and L-valine, respectively. Under normal circumstances, urine contains predominantly R-BAIB (from thymine) because it is eliminated by filtration and tubular secretion, while plasma contains about 80% S-BAIB (from valine), which is reabsorbed in the kidneys. In patients with dihydropyrimidine dehydrogenase deficiency, thymine catabolism is decreased, which results in moderately decreased CSF concentration and markedly decreased plasma concentration of BAIB. On the other hand, urine BAIB is elevated despite decreased breakdown of thymine, which indicates that there must be cross-over in the thymine and valine degradation pathways. Creatinine or creatine anhydride, is a breakdown product of creatine phosphate in muscle. The loss of water molecule from creatine results in the formation of creatinine. It is transferred to the kidneys by blood plasma, whereupon it is eliminated by glomular filtration and partial tubular excretion. Creatinine is usually produced at a fairly constant rate and measuring its serum level is a simple test. A rise in blood creatinine levels is observed only with marked damage to functioning nephrons; therefore this test is not suitable for detecting early kidney disease. Creatine and creatinine are metabolized in the kidneys, muscle, liver and pancreas.

D-serine (D-ser) is a major endogenous NMDA receptor co-agonist that specifically activates pre-synaptic NMDA receptors. D-ser is produced from L-serine by the enzyme serine racemase. Altered D-ser concentration may be associated with a number of central nervous disorders, including schizophrenia, aging, Alzheimer's disease, anxiety, cerebellar ataxia, Parkinson's disease, neuropathic pain and depression. Increasing the level of CNS D-ser by direct supplementation, or increasing production or decreasing breakdown of D-ser is being studied as a treatment option for conditions such as schizophrenia and Parkinson's disease.

L-serine (L-ser) is an amino acid that plays a central role in cellular proliferation and is the major source of one-carbon groups for the de novo synthesis of purine nucleotides and deoxythymidine monophosphate. L-ser is converted to D-ser by serine racemase. Altered CNS D-ser levels have been associated with a variety of neurologic conditions, including schizophrenia and Parkinson's disease. L-ser also plays a specific role in the CNS, and patients with defects of L-ser synthesis have significant neurological impairment. Two enzyme defects linked to L-ser synthesis have been discovered: 3-phosphoglycerate dehydrogenase (3-PGDH) deficiency and phosphoserine phosphatase (PSP) deficiency. Patients with 3-PGDH deficiency can have significant neurologic impairment, including congenital microcephaly and seizures associated with decreased white matter volume and myelination, and L-ser supplementation has been used to treat the seizures in this patient population. PSP deficiency has only been identified in one patient, whose symptoms also improved with L-ser treatment. PMID: 12534373
