LP229979-2 Time to end-systole
Left ventricular (LV) systolic dyssynchrony represents variability in the timing of contraction in different LV myocardial segments and is an indicator of LV systolic dysfunction. LV dyssynchrony is evaluated using cardiac gated SPECT and is based on time to end-systole (TES) measurements for each of the 17 LV segments. The TES measures the time from the end of diastole, which is the same for all segments, to the end of systole, which varies across segments depending on the degree of LV dysfunction.
The maximum difference in time to end-systole (MDTES), is the lag between the earliest and the latest end-systole among the 17 LV segments. The standard deviation of time to end-systole (SDTES) is the standard deviation of TES among all of 17 LV segments. Increasing MDTES and SDTES values, which represent increasing LV dyssynchrony, are associated with decreased LV ejection fraction and LV dysfunction. PMID: 22459387
The difference in time to end-systole between the lateral and septal segments (DTES-LS) is also derived from the TES. MDTES, SDTES and DTES-LS are typically expressed as percentages. PMID: 26860109

82625-5 Time to peak filling rate Left ventricle SPECT --resting
Part Descriptions
LP229982-6 Time to peak filling rate
Left ventricular (LV) diastolic filling parameters are based on analysis of LV volume over time as measures of LV diastolic function. The peak filling rate (PFR) is the maximum change in LV volume over time (dV/dt) during diastole. The one-third mean filling rate (1/3 MFR) is the average filling rate over the first third of diastole. The time to PFR (TPFR) is the duration of time from the end of systole to the time where the PFR occurs. The ratio of TPFR to R-R interval is the ratio of the time to peak filling to the total time of one cardiac cycle. In one study, patients with the connective tissue disorder systemic sclerosis were found to have significantly higher TPFR and TPFR/R-R interval values compared to controls. PMID: 11216514 Another study of patients with hypertension found that 1/3 MFR measured after stress was significantly lower in patients with hypertension compared to controls, as was the stress PFR. 82638-8 Phase entropy [Score] Left ventricle SPECT --resting
Part Descriptions
LP230000-4 Phase entropy
Phase analysis of the timing of myocardial contraction is used in nuclear cardiology to quantify regional ventricular wall motion and evaluate certain conditions, including bundle branch block, ventricular pacing, and pre-excitation syndromes. Phase and amplitude together are used to classify wall motion, which is also a measure of cardiac dyssynchrony.
The phase value is defined as the amount of shift in the sine or cosine curve (measured in degrees) using the fundamental frequency of the Fourier fitting, and all of the individual phase values are used together to create a phase histogram in order to see the distribution pattern of phase values. Separate histograms are created for the rest and stress portions of the study.
Multiple parameters are calculated from the histogram:
1. Phase maximum (degrees);
2. Phase mean (degrees);
3. Phase standard deviation (degrees);
4. Phase bandwidth, which is defined as 95% of the histogram width (degrees); and
5. Phase entropy, which is an index of disorder reported on a scale of 0-1 (0-100%), with 0 meaning complete order and 1 meaning complete disorder.
