Congenital cytomegalovirus (CMV) infection is a significant cause of infant morbidity and mortality. This test determines the presence or absence of CMV DNA in saliva, and can be used for testing newborns for CMV infection.

LP14255-1 Cytomegalovirus
Cytomegalovirus is a member of the family Herpesviridae and also known as Human Herpesvirus-5 (HHV-5). It contains a linear double-stranded DNA encased in an icosahedral capsid that is surrounded by a viral envelope. It replicates slowly and may take weeks for the infection to become visible microscopically. Its life cycle includes a lytic and a latent phase. During the lytic phase the cytoskeleton of the infected cell is disrupted and becomes very large. The enlarged cell eludes detection by natural killer (NK) cells by encoding UL16. UL16 is a protein that binds to the ligand NDG2D which is a surface receptor on NK cells and, when blocked, prevents the CMV infected cell from apoptotic signaling to NK cells to destroy it.[https://pedclerk.bsd.uchicago.edu/page/cytomegalovirus-cmv] In the US one in three children are infected by CMV by the age of five and over half of adults are infected by age 40. Infection with CMV is mostly asymptomatic. Symptoms when present include fever, sore throat, fatigue, swollen glands. CMV can occasionally lead to mononucleosis or hepatitis. Immunocompromised patients experience much more acute symptoms that can affect the eyes, lungs, liver, esophagus, stomach, and intestines. For babies born with CMV, involvement with the brain, spleen, lung, liver can occur. In addition, there may be growth problems and hearing loss which is the most common symptom in congenital CMV infection. CMV is transmitted in urine, saliva, blood, tears, semen and breast milk by direct contact. Normally no treatment is necessary, but for severe cases medication is available.[https://www.cdc.gov/cmv/overview.html]

Saliva is produced primarily by the parotid, submaxillary, and sublingual glands. It is secreted at the rate of 0-3 milliliters per minute which is affected by various factors including anxiety, hydration and hunger. Oral fluid is broadly used to test for drugs of abuse in employment and forensic settings, and is especially useful since it is difficult to adulterate, is quick and is non-invasive. Oral fluid is used to test a variety of analytes, including to test alcohol, HIV and other antibodies, therapeutic drugs and steroids, but its primary use is to test for drugs of abuse including amphetamines, cocaine and metabolites, opioids, methadone, cannabis and heroin.
