Neisseria meningitides (Nm) is a species of gram-negative, aerobic bacteria. It is a commensal and pathogen only of humans, and can be carried asymptomatically in the nasopharynx. When found in cerebrospinal fluid it is the causative agent of cerebrospinal meningitis (meningitis, meningococcal). Nm can also be found in venereal discharges and blood. At least 13 serogroups have been described based on antigenic differences in the capsular polysaccharides; the ones causing most meningitis infections being A, B, C, Y, and W-135. Outbreaks caused by serogroup X meningococci have also been reported. Each serogroup can be further classified by serotype, serosubtype, and immunotype. (NLM, MeSH 2006) The two targets most frequently used in real-time PCR assays to identify Nm are the capsule transport gene (ctrA) and the Cu-An superoxide dismutase gene (sodC). sodC is found only in Nm and not in other species of Neisseria. sodC is highly sensitive and specific for the detection of Nm regardless of whether capsule genes are present or absent. ctrA is likely the target gene most often used to identify Nm, but because it is prone to rearrangement it does not detect roughly 16% of patients with Nm.
