Red blood cells (RBCs) have surface antigens that play a role in determining compatibility for blood transfusions and organ transplants. The common antigens on RBCs are from the ABO system and include A, B and H antigens. The H antigen is a carbohydrate on the RBC surface which is the biosynthetic precursor required for the expression of the A and B antigens. Cells that express H antigen and lack A and B antigens are, by default, in blood group O. In rare cases, the H antigen is not expressed on the red cell membrane at all, which is described as having an O sub h or Bombay phenotype. Individuals with the Bombay phenotype produce antibodies to the A, B and H antigens, which antibodies are capable of activating complement and causing hemolysis. The only blood that a patient with Bombay phenotype may receive is that from another person who also expresses the Bombay phenotype. (AABB Technical Manual, 18th edition)
