This test is used for the detection of vancomycin resistance genes by multiplex PCR. Vancomycin is a glycopeptide antibiotic used to treat various infections, primarily those caused by gram-positive bacteria. Vancomycin resistance genes were first found in enterococci, and more recently have been found in Staphylococcus aureus. The emergence of vancomycin-resistant enterococci (VRE) and vancomycin-resistant Staphylococcus aureus (VRSA) in hospitalized patients has lead to a restricted use of vancomycin. To date, multiple vancomycin resistance genes have been characterized, including: vanA, vanB, vanC (vanC-1, vanC-2, vanC-3), and VanD. Of the six van types, vanA and vanB are the most resistant to vancomycin, and are the two types that cause VRSA. PMID: 24983424

Non-susceptibility to carbapenems in gram-negative bacilli by means of the enzyme KPC (Klebsiella pneumoniae carbapenemase) is becoming more common. The gene beta-lactamase KPC (bla(KPC)) encodes KPC enzyme production, which in turn hydrolyzes and inactivates carbapenem antibiotics. KPC enzymes are a class A beta lactamases with several variants (KPC-2, KPC-3, etc) known. Various methods are used to detect specific portions of genes encoding KPC production. Carbapenemases can also be detected by other methods, including the modified Hodge test to detect carbapenemase production in isolates and minimum inhibitory concentration (MIC) to determine level of resistance.

Previously known as Torulopsis glabrata, Candida glabrata is a fungal saprophyte that is part of the normal flora of human mucosal tissues and is usually nonpathogenic. Unlike other species of Candida, C glabarata has a nondimorphic blastoconicial morphology and a haploid genome. C glabrata often is implicated in superficial infections of the mouth, esophagus, vagina, urinary tract, and in systemic candida infections. C glabrata infection has become common in patients on immunosuppressive therapy and in patients with diabetes. It can be difficult to treat since it is resistant to a number of antifungal agents including fluconazole. As a result, immunocompromised patients have a high mortality rate when infected with C glabrata. PMID: 9880475

Candida krusei is an elongated facultative saprophyte of variable shape and size. It takes on both yeast and pseudohyphae morphological forms. It is found in various natural habitats including in the atmosphere, fruits, sewage, silage, soil, dairy, meat, pickles, sugar, wine, and beer. Serious infections are seen in immunocompromised patients with hematologic malignancies, especially leukemia, and in transplant recipients. A rise in the number of infections with C krusei is associated with the spread of the human immunodeficiency virus (HIV) and the use of the antifungal triazole fluconazole. C krusei is also the cause of infection due to invasive medical procedures performed on immunocompromised patients that can cause endophthalmitis, arthritis, and endocarditis. Mortality rates due to C. krusei

Candida parapsilosis is a commensal fungus of human skin and a major cause of infection in immunocompromised and hospitalized patients with serious comorbidities. It is oval, round, or cylindrical in shape, and exists as a yeast or pseudohyphae. Infection with C parapsilosis in humans has increased significantly over the past 10 years, and is especially dangerous to critically ill neonates, that experience more than 25% of all invasive fungal infections in low-birth weight infants in the United Kingdom, and nearly one-third of neonatal Candida bloodstream infections in North America. C parapsilosis is known to be spread by hand contact in the hospital environment, but can also form biofilms on catheters and other implant devices and spread from there. It is not an obligate human pathogen and has been isolated from domestic animals, insects, soil, and marine environments. PMID: 18854483

Candida tropicalis is a significant species of Candida with regard to its epidemiology and virulence. It produces a number of virulence factors including biofilm, adhesion to buccal epithelial and endothelial cells, the secretion of lytic enzymes, bud-to-hyphae transition, and phenotypic switching. In Asia and Latin America, C tropicalis is the second or third most common species of Candida to cause systemic disease. It is able to survive in high salt concentrations, that is significant for fungal persistence in saline environments. C tropicalis is resistant to several antifungal drugs currently in use including the azole derivatives, amphotericin B, and the echinocandins. It exists as a true hyphae and diploid yeast that cannot be distinguished from other Candida species on Sabouraud Dextrose Agar (SDA). Enterococcus is a genus of lactic acid bacteria of the phylum Firmicutes. Enterococci are Gram-positive cocci that often occur in pairs (diplococci) or short chains, and are difficult to distinguish from streptococci on physical characteristics alone.Two species are common commensal organisms in the intestines of humans: E. faecalis (90-95%) and E. faecium (5-10%). Rare clusters of infections occur with other species, including E. casseliflavus, E. gallinarum, and E. raffinosus.
Important clinical infections caused by Enterococcus include urinary tract infections, bacteremia, bacterial endocarditis, diverticulitis, and meningitis.Sensitive strains of these bacteria can be treated with ampicillin and vancomycin.
From a medical standpoint, an important feature of this genus is the high level of intrinsic antibiotic resistance. Some enterococci are intrinsically resistant to β-lactam-based antibiotics (penicillins,cephalosporins,carbapenems), as well as many aminoglycosides. In the last two decades, particularly virulent strains of Enterococcus that are resistant to vancomycin (vancomycin-resistant Enterococcus, or VRE) have emerged in nosocomial infections of hospitalized patients, especially in the US. Other developed countries, such as the UK, have been spared this epidemic, and, in 2005, Singapore managed to halt an epidemic of VRE.[citation needed] VRE may be treated with quinupristin/dalfopristin (Synercid) with response rates of approximately 70%. Tigecycline has also been shown to have anti-enterococcal activity as has rifampicin.
Enterococcal meningitis is a rare complication of neurosurgery. It often requires treatment with intravenous or intrathecal vancomycin, yet it is debatable as to whether its use has any impact on outcome: the removal of any neurological devices is a crucial part of the management of these infections. New epidemiological evidence has shown that enterococci are major infectious agent in chronic bacterial prostatitis. Enterococci are able to form biofilm in the prostate gland making their eradication difficult.

Escherichia coli (EC) is a gram-negative, flagellated, rod-shaped bacterium. The natural barriers of the GI tract prevent infection by symbiotic EC except when they break down such as in peritonitis. EC occupy the mucous layer of the mammalian colon and is the most numerous anaerobe of the intestinal microflora. Some strains of EC have evolved virulence factors that precipitate serious disease including enteric diarrhea, urinary tract infections, sepsis and meningitis. The intestinal pathogens of EC include six pathotypes defined by the virulence factors they express. These include: enteropathogenic E. coli (EPEC), enterohaemorrhagic E. coli(EHEC), enterotoxigenic E. coli (ETEC), enteroaggregative E. coli (EAEC), enteroinvasive E. coli (EIEC) and diffusely adherent E. coli (DAEC). Uropathogenic EC (UPEC) is the most common EC infection that occurs outside the GI tract. The EC pathotypes that infects extraintestinal locations have been designated ExPEC. Haemophilus influenza is a pleomorphic gram-negative coccobacillus. H influenzae is divided into six serotype (A-F) based on their capsular polysaccharides. The majority of haemophilus infections are caused by H influenza serotype B (HIB) which causes pneumonia, bacteremia, meningitis, epiglottitis, septic arthritis, cellulitis, otitis media, purulent pericarditis, endocarditis and osteomyelitis. Infections with the remaining serotypes lead to similar disease processes as HIB. Unencapsulated H influenzae that is not able to be typed (nontypable H influenzae) can cause ear infections in children and bronchitis in adults but also bacteremia and bacteremic pneumonia. Nontypable H influenza is the main cause of invasive H influenzae infections in all age groups. Transmission of H influenzae is by direct contact with respiratory aerosols. Klebsiella pneumoniae is part of the Enterobacteriaceae family and is a gram-negative, encapsulated and non-motile bacterium. Humans are the primary carrier of K. pneumonia. Diagnosis is made through a combination of laboratory analysis, and chest x-ray imaging, for example, lobar infiltrate in the posterior aspect of right upper lung and/or bulging fissure sign. K. pneumonia is carried in 5-38% and 1-6% of the general population's stool and nasopharynx, respectively. Up to 77% of hospitalized patients are carriers. PMID: 30085546

Neisseria meningitidis is a species of gram-negative, aerobic bacteria. It is a commensal and pathogen only of humans, and can be carried asymptomatically in the nasopharynx. When found in cerebrospinal fluid it is the causative agent of cerebrospinal meningitis (meningitis, meningococcal). It can also be found in venereal discharges and blood. At least 13 serogroups have been described based on antigenic differences in the capsular polysaccharides; the ones causing most meningitis infections being A, B, C, Y, and W-135. Outbreaks caused by serogroup X meningococci have also been reported. Each serogroup can be further classified by serotype, serosubtype, and immunotype.

Serratia marcescens is an opportunistic, gram negative, pathogen from the family Enterobacteriaceae that causes both opportunistic and nosocomial infections. It is estimated that only 1-2% of hospital acquired infections, typically of the respiratory tract, urinary tract, surgical wounds or soft tissues, are due to Serratia marcescens. Serratia-associated meningitis can occur in pediatric patients. Diagnosis of Serratia marcescens infection is made through standard laboratory testing of blood, urine, pus and other bodily fluids. Staphylococci, members of the family Micrococcaceae, are Gram-positive cocci that grow in irregular grape-like clusters. Traditionally, Staphylococci are divided into two groups: coagulase-positive and coagulase-negative Staphylococci (CoNS). The coagulase-positive group includes the well-known pathogen S. aureus, whereas the coagulase-negative group includes a number of species that are known commensals on skin. It is clear that even the CoNS are true pathogens, especially in patients with indwelling devices, and given their prevalence, are common causes of bacteremia. S. aureus is known for causing a number of superficial skin infections (boils, furuncles, impetigo, etc), but it can also cause more serious infections, especially in immunosuppressed patients including pneumonia, deep abscesses, osteomyelitis, endocarditis, and meningitis. Multiple antibiotic resistance is increasingly common in both S. aureus and CoNS, with the emergence of methicillin-resistant S. aureus (MRSA) able to reach epidemic levels. Staphylococcus aureus (S. aureus) are cocci-shaped gram-positive bacteria that assemble in grape-like clusters and cause a wide variety of infections. Up to 50% of adults carry S. aureus on their skin or in their mucous membranes. It is highly adaptable, resulting in drug resistant strains: methicillin-resistant S. aureus (MRSA), methicillin-susceptbile S. aureus (MSSA), vancomycin-intermediate S. aureus (VISA) and vancomycin-resistant S. aureus (VRSA). Examples of human infections caused by S. aureus: bacteremia, gastroenteritis, infective endocarditis, meningitis, osteomyelitis, pulmonary infections, septic arthritis, skin and soft tissue infections, toxic shock syndrome, and urinary tract infections. Diagnosis of S. aureus infection may vary by infection site, e.g., clinical signs and symptoms for skin infections, blood or tissue cultures for other infection types. People with chronic health conditions, weakened immune systems or IV drug users are at a greater risk, as are patients in intensive care units (ICUs) and those with a history of certain types of surgeries and patients with implanted medical devices. Streptococcus pneumoniae are gram-positive cocci that are nonmotile and nonsporulating and usually require complex culture media. Older streptococcal cultures, beyond the most active growth period (the logarithmic phase), sometimes lose their gram-positive staining characteristics. S. pneumoniae are part of the normal respiratory tract flora. Disease usually results due to the acquisition of a new strain following a change in the normal flora, often due to a preceding virus. S. pneumonaie causes otitis media, sinusitis and pneumonia in the middle ear, sinuses or lungs, respectively. Meningitis can occur by bloodstream invasion from the lungs or by extension of otitis media or sinusitis to the meninges. Bacteremia can result in osteomyelitis (infection of bones) or arthritis (of joints). Most bacterial pneumonia occurring after the neonatal period is caused by S. pneumoniae. Pneumococcal pneumonia is seen most commonly in elderly or immunosuppressed individuals. In the United States, it is most prevalent in winter. Diagnosis is usually based on a sputum Gram stain and culture although blood or cerebrospinal fluid may be cultured. While penicillin has been the standard treatment, some strains have proven resistant to penicillin and multiple antibiotics. A vaccine exists. [NCBI Books:NBK7611]

Salmonella are gram-negative, flagellated, facultative intracellular, anaerobic bacilli with O, H and Vi surface antigens. Once ingested, Salmonella survives the gastric acid barrier and penetrates the epithelial cells of the small and large intestines. The invasion of the epithelial cells stimulates the release of cytokines and a subsequent inflammatory response leading to diarrhea and possible ulceration of the mucosa.[NCBI Books: 8435] Antibodies to Salmonella are the body's defense when the bacteria is outside of its host cell which occurs at the initial infection and when the bacillus moves from cell to cell. There is a 10 minute window of opportunity when the Salmonella bacillus leaves the macrophage to invade another. During that time, antibodies and complement work to eradicate the bacilli, but some of the extracellular bacilli will evade attack and enter another host cell. Studies have shown that vaccines play an important role in the fight against typhoid fever. PMID: 25566248 There are two species of Salmonella: S. enterica and S. bongori. These species can be further subclassified into subspecies and serovars.
Veterinary Medicine:
Salmonella infection is most often caused by the serovars Salmonella choleraesuis, and Salmonella typhimurium and less often by Salmonella derby. Infected pigs can be asymptomatic and become carriers of S. choleraesius. Typically, the bacteria survive in the mesenteric lymph nodes that drain into the intestines, and are not shed in the feces unless the pig is under stress. The carrier state lasts for weeks to months and is self-limiting. Severe infection with S. choleraesius can be serious starting out as septicemia and progressing to pneumonia and enteritis. The infection can spread to various tissues including to the central nervous system leading to meningitis and to the joints where it manifests as arthritis. Stenotrophomonas maltophilia is an aerobic, nonfermentative, Gram-negative bacterium. It is an uncommon bacterium and human infection is difficult to treat. Initially classified as Pseudomonas maltophilia, S. maltophilia was also grouped in the genus Xanthomonas before eventually becoming the type species of the genus Stenotrophomonas in 1993.
S. maltophilia are slightly smaller than other members of the genus. They are motile due to polar flagella and grow well on MacConkey agar producing pigmented colonies. S. maltophilia are catalase-positive, oxidase-negative (which distinguishes them from most other members of the genus) and have a positive reaction for extracellular DNase.
S. maltophilia is ubiquitous in aqueous environments, soil and plants, including water, urine, or respiratory secretions; it has also been used in biotechnology applications. In immunocompromised patients, S. maltophilia can lead to nosocomial infections.

Candida auris is a fungus that was first identified in 2009 in Japan and can cause serious infections in the blood, spinal fluid, and internal organs. This emerging fungus has spread quickly and infections have been documented all over the world. C auris is difficult to identify with the currently available phenotypic methods used by laboratories, and can be misidentified as other species of yeast. It is often resistant to multiple drugs, though resistance can differ broadly across isolates. There are currently no established breakpoints to determine susceptibility, so laboratories have relied on the breakpoints of closely related Candida species or the opinion of experts in mycology. In the United States, C auris is resistant to fluconazole in 90% of infections, to amphotericin B in 30% of infections, and to echinocandins in 5% of infections. Patients at highest risk of infection are those that have been hospitalized for a long time, those with indwelling catheters, and those who have been previously treated with antibiotic or antifungal medication. LP249101-9 Bacterial colistin resistance mcr-1 gene
The mobilized colistin resistance genes (mcr-1 to mcr-5, and variants) are located on a plasmid within bacterial cells and affect the ability of antibiotics, including polymyxins and colistin, to bind to the bacteria. The mode of action of the antibiotics is to bind to the outer membrane of the phosphate group of lipid A lipopolysaccharide and to intercalate into the inner membrane forming pores that result in bacterial lysis. The polymyxins & colistin are the last line of defense used to treat multidrug resistant gram-negative bacteria so the emergence of the mcr genes in Enterobacteriaceae, including Klebsiella pneumoniae, is onerous and challenging to public health.
