Aminoacyl-tRNA synthetases (ARS) are enzymes that link amino acids to tRNA molecules and therefore play a key role in protein synthesis. Each ARS is specific for a single amino acid. [NCBI Books: NBK22356] Autoantibodies to ARS are a hallmark of connective tissue diseases including anti-synthetase syndrome (ASS) and idiopathic inflammatory myopathies such as polymyositis (PM) and dermatomyositis (DM). ASS, PM and DM have overlapping features, including myositis, arthralgia, skin manifestations and interstitial lung disease. Antibodies to several specific ARS enzymes have been identified, including anti-histidyl (anti-Jo-1), anti-threonyl (anti-PL-7), anti-alanyl (anti-PL-12), anti-glycyl (anti-Ej), anti-isoleucyl (anti-Oj), anti-asparaginyl (anti-Ks), anti-phenylalanyl (anti-Zo), and anti-tyrosyl (anti-Ha). Different antibodies or patterns of antibodies have been associated with specific clinical features. Anti-Jo-1, anti-Ej, and anti-PL-7 antibodies are associated with myositis, including classic DM, clinically amyopathic DM (CADM) and PM. Anti-PL-12 alone is associated with CADM or interstitial lung disease (ILD). Anti-Ej and anti-PL-12 together are associated with CADM. Anti-Ks and anti-Oj are associated with ILD alone. Histones are small nuclear proteins that together with dsDNA, represent a nucleosome, the structural unit of chromatin. Anti-histone antibodies are associated with various autoimmune diseases, including systemic lupus erythematosus (SLE), rheumatoid arthritis, Sjogren's syndrome, systemic sclerosis, infectious diseases (including HIV), and some neurological disorders such as Alzheimer's disease and dementia. Nearly all patients with drug-induced SLE have anti-histone antibodies, but they are also present in patients with idiopathic forms of the disease. One study showed a correlation between SLE with positive anti-dsDNA, anti-nucleosome and anti-histone antibodies and renal disease, including proliferative glomerulonephritis. PMID: 24649358

If antibodies to any of those antigens are present in the patient's serum, they will bind to the specific antigen. The bound patient antibodies are then detected using labelled anti-human antibodies. Manufacturers call this method by different names, including immunoblot, line blot, and line immunoassay. LOINC uses the immunoblot (IB) method to mean that some electrophoretic/chromatographic separation occurs such that position (size) of the band is part of the assessment (as in Western blot). Because this assay does not involve a separation step but rather uses strips with pre-fixed antigens, the LOINC Method for this term is Line blot.

It translates Messenger RNA (mRNA) into a polypeptide chain (e.g., a protein). It can be thought of as a factory that builds a protein from a set of genetic instructions. Ribosomes can float freely in the cytoplasm (the internal fluid of the cell) or bind to the endoplasmic reticulum, or to the nuclear envelope. Ribosomes were first observed in the mid-1950s by cell biologist George Palade in the electron microscope as dense particles or granules for which he would win the Nobel Prize. Topoisomerase I (also known as Scl-70) is a nuclear enzyme that plays a role in releasing tension that is introduced in double-stranded DNA during DNA replication by transiently cleaving and rejoining one strand of the DNA duplex. Anti-Scl-70 autoantibodies are a marker of systemic sclerosis as well as other autoimmune diseases, including systemic lupus erythematosus (SLE). In patients with systemic sclerosis, anti-SCL-70 may be associated with more severe disease, and in patients with SLE, anti-SCL-70 suggests an increased risk of pulmonary hypertension and renal involvement. In general, anti-SCL-70 antibody status may help with diagnosis, treatment planning and prognosis in patients with connective tissue disease. PMID: 11229469

Sjogren syndrome type A antigen (also known as SS-A, Ro, and E3 ubiquitin-protein ligase TRIM21) and Sjogren syndrome type B antigen (also known as SS-B and Lupus La protein) are nuclear antigens to which autoantibodies develop in autoimmune disorders such as Sjogren's syndrome (SS) and systemic lupus erythematosus (SLE). Between 40 and 70% of patients with SS have anti-SSA and/or anti-SSB autoantibodies, and 30-40% and 10-15% of patients with SLE are anti-SSA and anti-SSB positive, respectively. Longitudinal studies have shown that anti-SSA and -SSB antibodies may be present up to 20 years before the clinical onset of disease. SS patients who are anti-SSA and anti-SSB positive may have longer disease duration, parotid gland enlargement, decreased salivary flow, more severe minor salivary gland infiltration and a higher prevalence of extraglandular manifestations. Term Description
This term was created for, but is not limited in use to, the submitter's test strip, a membrane strip pre-coated with a specific set of antigens in parallel lines. If antibodies to any of those antigens are present in the patient's serum, they will bind to the specific antigen. The bound patient antibodies are then detected using labelled anti-human antibodies. Various manufacturers classify this method as an immunoblot technique. LOINC uses the immunoblot (IB) method to mean that some electrophoretic/chromatographic separation occurs such that position (size) of the band is part of the assessment (as in Western blot). Because this assay does not involve a separation step but rather uses strips with pre-fixed antigens, the LOINC Method is Line blot.

U1 small nuclear ribonucleoprotein particle (U1 snRNP) is an RNA-protein complex that is composed of ten proteins: U1-70kD, U1A, U1C and seven Smith antigen (Sm) proteins. The U1 snRNP complex is responsible for pre-mRNA processing. Autoantibodies to different components of the U1 snRNP complex are associated with autoimmune conditions including systemic lupus erythematosus (SLE) and mixed connective tissue disorders (MCTD), which includes systemic sclerosis. The pattern of autoantibodies present in an individual patient may help distinguish between different connective tissue disorders. For example, patients with anti-U1A and anti-U1C IgM antibodies are more likely to have SLE than MCTD. PMID: 24158973 In addition, there is some evidence that specific antibody patterns are associated with specific symptoms. Another study found an association between the number of specific positive antibodies within the anti-U1 snRNP (rather than the presence of particular markers) and certain clinical features. For example, patients with three or more positive autoantibodies were more likely to have features of systemic sclerosis and less likely to have renal disease compared to patients with fewer types of antibodies.
