Epithelial casts are composed of epithelial cells from the renal tubule lining. The presence of epithelial casts in urine reflects damage to tubule cells in the kidney. They are associated with acute tubular necrosis, viral disease such as cytomegalovirus, kidney transplant rejection and exposure to nephrotoxic substances such as mercury and ethylene glycol.
