Barbiturates are CNS depressants that primarily act on gamma-aminobutyric acid (GABA) and GABA A receptors. When GABA binds to GABA A, chloride channels open and allow the influx of chloride ions, which decrease the membrane potential and inhibit the neuron's action potential. They are used as sedatives, hypnotics, anesthetics, and anticonvulsants. Prolonged use can lead to physical tolerance and dependence, which has caused a significant decrease in their use over time. However, certain uses are still common, such as phenobarbital for refractory seizure disorders, butalbital for analgesia, and thiopental for anesthesia. PMID: 25824249

The group of drugs called cannabinoids includes both the cannabinoid chemicals found in the cannabis plant as well as synthetic and pharmaceutical cannabinoids. The cannabinoids in cannabis include tetrahydrocannabinol (THC), cannabidiol (CBD), and cannabinol (CBN). THC is responsible for the psychoactive effects of cannabis and binds to the CB1 and CB2 cannabinoid receptors. CBD does not have psychoactive effects but rather has a variety of therapeutic properties including anticonvulsant, analgesic, anti-anxiety, and antiemetic. The THC to CBD ratio determines the physiologic effects of the drug. A high THC/CBD ratio results in euphoric and anxiolytic effects, while a low THC/CBD ratio results in sedative effects. Both natural and synthetic cannabinoid compounds are used for therapeutic purposes. Synthetic cannabinoids, like THC, act on the CB1 and CB2 receptors. Adverse reactions, including psychosis, confusion, agitation, tachycardia, myocardial infarction, seizures, and fatal overdoses, are more common with synthetic cannabinoids compared to cannabis. Cocaine is an alkaloid of Erythroxylon coca that is grown in the northern South American Andes, India, Africa and Java. It is used medicinally as a local anesthetic and vasoconstrictor in nasal surgery and to dilate pupils. Because cocaine is a powerful central nervous system stimulant it is among the most abused drugs. Cocaine abuse can lead to sudden death due to cardiotoxicity or the sequential development of hyperthermia, agitated delirium and respiratory arrest. Cocaine is metabolized in the liver to inactive benzoylecgonine (BZE). BZE is pharmacologically inactive and has a longer half-life than its parent drug cocaine. It is the substance most tested for when investigating suspected cocaine abuse. Methamphetamine (MA) is a drug of abuse also referred to as meth, crystal and speed. It is a central nervous system stimulant that can be injected, smoked, snorted or ingested. Short-term MA abuse causes an increase of energy, sense of well-being and decreased appetite that lasts 6-8 hours. PMID: 19042205 MA use can lead to stroke, cardiac arrhythmia, stomach cramps, shaking, anxiety, insomnia, paranoia, hallucinations, and structural changes to the brain. Pregnant women who use MA can cause growth retardation, premature birth, and developmental disorders in neonates and long lasting intellectual deficits to the children as they grow. PMID: 10908000 Users addicted to MA experience withdrawal symptoms that include dysphoria, fatigue, insomnia and an increased appetite. In chronic MA users positron emission tomography has proven a decrease in dopamine D2 receptors which is associated with mental deterioration that can be reversed to some extent after abstaining from MA. Methylenedioxymethamphetamine (MDMA) (3,4-methylenedioxymethamphetamine) is an empathogenic drug of the phenethylamine and amphetamine classes of drugs. Amphetamines and phenethylamines are a class of chemicals/drugs that are synthetic, psychoactive drugs. Many are sympathomimetics and central nervous system stimulators causing excitation, vasopressin, bronchodilation, and to varying degrees, anorexia, analepsis, nasal decongestion, and some smooth muscle relaxation. In 1985, the US Drug Enforcement Administration moved the drug to Schedule 1 status with no accepted medical use.

Methamphetamine (MA) is a drug of abuse also referred to as meth, crystal and speed. It is a central nervous system stimulant that can be injected, smoked, snorted or ingested. Short-term MA abuse causes an increase of energy, sense of well-being and decreased appetite that lasts 6-8 hours. PMID: 19042205 MA use can lead to stroke, cardiac arrhythmia, stomach cramps, shaking, anxiety, insomnia, paranoia, hallucinations, and structural changes to the brain. Pregnant women who use MA can cause growth retardation, premature birth, and developmental disorders in neonates and long lasting intellectual deficits to the children as they grow. PMID: 10908000 Users addicted to MA experience withdrawal symptoms that include dysphoria, fatigue, insomnia and an increased appetite. In chronic MA users positron emission tomography has proven a decrease in dopamine D2 receptors which is associated with mental deterioration that can be reversed to some extent after abstaining from MA.
