The Enterobacter cloacae complex comprises of six phenotypically and genetically heterogeneous species: Enterobacter cloacae, Enterobacter asburiae, Enterobacter hormaechei, Enterobacter kobei, Enterobacter ludwigii and Enterobacter nimipressuralis. Among the Enterobacter spp., the E. cloacae complex species are the most common cause of nosocomial bloodstream infections, with E. cloacae and E. hormaechei most frequently isolated in human clinical specimens. The emergence and spread of antimicrobial resistance in the E. cloacae complex has increased the complexity of treating infections due to these organisms. Escherichia coli (EC) is a gram-negative, flagellated, rod-shaped bacterium. [https://vetmed.iastate.edu/vdpam/FSVD/swine/index-diseases/colibacillosis] The human gastrointestinal (GI) tract is colonized by EC within hours of birth. The natural barriers of the GI tract prevent infection by symbiotic EC except when they break down such as in peritonitis. EC occupy the mucous layer of the mammalian colon and is the most numerous anaerobe of the intestinal microflora. Some strains of EC have evolved virulence factors that precipitate serious disease including enteric diarrhea, urinary tract infections, sepsis and meningitis. The intestinal pathogens of EC include six pathotypes defined by the virulence factors they express. These include: enteropathogenic E. coli (EPEC), enterohaemorrhagic E. coli(EHEC), enterotoxigenic E. coli (ETEC), enteroaggregative E. coli (EAEC), enteroinvasive E. coli (EIEC) and diffusely adherent E. coli (DAEC). Uropathogenic EC (UPEC) is the most common EC infection that occurs outside the GI tract. The EC pathotypes that infects extraintestinal locations have been designated ExPEC. Escherichia coli (EC) is a gram-negative, flagellated, rod-shaped bacterium. [https://vetmed.iastate.edu/vdpam/FSVD/swine/index-diseases/colibacillosis] The human gastrointestinal (GI) tract is colonized by EC within hours of birth. The natural barriers of the GI tract prevent infection by symbiotic EC except when they break down such as in peritonitis. EC occupy the mucous layer of the mammalian colon and is the most numerous anaerobe of the intestinal microflora. Some strains of EC have evolved virulence factors that precipitate serious disease including enteric diarrhea, urinary tract infections, sepsis and meningitis. The intestinal pathogens of EC include six pathotypes defined by the virulence factors they express. These include: enteropathogenic E. coli (EPEC), enterohaemorrhagic E. coli(EHEC), enterotoxigenic E. coli (ETEC), enteroaggregative E. coli (EAEC), enteroinvasive E. coli (EIEC) and diffusely adherent E. coli (DAEC). Uropathogenic EC (UPEC) is the most common EC infection that occurs outside the GI tract. The EC pathotypes that infects extraintestinal locations have been designated ExPEC. A combination of serological and conventional microbiological studies is used to confirm infection by M. catarrhalis. M. catarrhalis may cause a variety of illnesses, ranging from sinusitis, otitis media, lower respiratory tract infections, in adultsconjunctivitis and keratitis in children, to laryngitis, bronchitis, pneumonia and lower respiratory tract infections, particularly in those with chronic obstructive pulmonary disease (COPD). A combination of serological and conventional microbiological studies is used to confirm infection by M. catarrhalis. Up to 75% of children carry M. catarrhalis, while carriage rates in adults is extremely low (1-3%). PMID: 11781271

Serratia marcescens is an opportunistic, gram negative, pathogen from the family Enterobacteriaceae that causes both opportunistic and nosocomial infections. It is estimated that only 1-2% of hospital acquired infections, typically of the respiratory tract, urinary tract, surgical wounds or soft tissues, are due to Serratia marcescens. Serratia-associated meningitis can occur in pediatric patients. Diagnosis of Serratia marcescens infection is made through standard laboratory testing of blood, urine, pus and other bodily fluids. It is highly adaptable, resulting in drug resistant strains: methicillin-resistant S. aureus (MRSA), methicillin-susceptbile S. aureus (MSSA), vancomycin-intermediate S. aureus (VISA) and vancomycin-resistant S. aureus (VRSA). Examples of human infections caused by S. aureus: bacteremia, gastroenteritis, infective endocarditis, meningitis, osteomyelitis, pulmonary infections, septic arthritis, skin and soft tissue infections, toxic shock syndrome, and urinary tract infections. Diagnosis of S. aureus infection may vary by infection site, e.g., clinical signs and symptoms for skin infections, blood or tissue cultures for other infection types. People with chronic health conditions, weakened immune systems or IV drug users are at a greater risk, as are patients in intensive care units (ICUs) and those with a history of certain types of surgeries and patients with implanted medical devices. Streptococcus pneumoniae are gram-positive cocci that are nonmotile and nonsporulating and usually require complex culture media. Older streptococcal cultures, beyond the most active growth period (the logarithmic phase), sometimes lose their gram-positive staining characteristics. S. pneumoniae are part of the normal respiratory tract flora. Disease usually results due to the acquisition of a new strain following a change in the normal flora, often due to a preceding virus. S. pneumonaie causes otitis media, sinusitis and pneumonia in the middle ear, sinuses or lungs, respectively. Meningitis can occur by bloodstream invasion from the lungs or by extension of otitis media or sinusitis to the meninges. Bacteremia can result in osteomyelitis (infection of bones) or arthritis (of joints). Most bacterial pneumonia occurring after the neonatal period is caused by S. pneumoniae. Pneumococcal pneumonia is seen most commonly in elderly or immunosuppressed individuals. In the United States, it is most prevalent in winter. Diagnosis is usually based on a sputum Gram stain and culture although blood or cerebrospinal fluid may be cultured. The bacterial (CTX-M) gene is targeted to detect CTX-M type beta lactamases, a group of class A extended-spectrum beta-lactamases (ESBLs) that are a common cause of community-acquired infections. They exhibit powerful activity against cefotaxime and ceftriaxoane. CTX-M type ESBLs are rapidly spreading among Enterobacteriaceae worldwide and replacing TEM-type and SHV-type beta lactamases as the predominant ESBLs in many countries. They originate from Kluyvera species and can be separated into 5 different groups: CTX-M-1, CTX-M-2, CTX-M-9, CTX-M-8, and CTX-M-25. CTX-M2, CTX-M3, and CTX-M14 are the most prevelant. More than 80 CTX-M allotypes are currently known (2014).

Plasmid-mediated IMP-type carbapenemases are currently known to have 17 varieties. Studies have revealed its establishment in Japan in the 1990s in enteric Gram-negative organisms, Pseudomonas species and Acinetobacter species. IMP genes usually co-exist with other resistance genes resulting in multi-drug resistance.
Studies have shown that a high percentage of patients with immunodeficiency and drug resistant nosocomial infections are infected with a VIM or IMP producing organism, particularly when the organism identified is pseudomonas aeruginosa. The two genes are often present together although they can also be present individually. Non-susceptibility to carbapenems in gram-negative bacilli by means of the enzyme KPC (Klebsiella pneumoniae carbapenemase) is becoming more common. The gene beta-lactamase KPC (bla(KPC)) encodes KPC enzyme production, which in turn hydrolyzes and inactivates carbapenem antibiotics. KPC enzymes are a class A beta lactamases with several variants (KPC-2, KPC-3, etc) known. Various methods are used to detect specific portions of genes encoding KPC production. Carbapenemases can also be detected by other methods, including the modified Hodge test to detect carbapenemase production in isolates and minimum inhibitory concentration (MIC) to determine level of resistance.

The mecA gene and its homologue, mecC, are found in bacterial cells and enable bacteria to be resistant to antibiotics such as penicillin, oxacillin and other penicillin-like antibiotics. These genes are carried on a mobile genetic element called the staphylococcal cassette chromosome mec (SCCmec), which integrates at an open reading frame (orfX) in the bacterial genome. The most commonly known carrier of the mecA gene is methicillin-resistant Staphylococcus aureus (MRSA). NDM-1 presents as the most dominant type. The NDM gene encodes beta-lactamase enzymes called carbapenemases. The most common bacteria that make the NDM enzyme are Gram-negative such as Escherichia coli and Klebsiella pneumoniae, but the gene for NDM can spread from one strain of bacteria to another by horizontal gene transfer.

The existence of OXA-48 like class D beta-lactamases has been increasing in enterobacter species organisms. The "OXA-like" groups of genes are molecularly similar to and thought to evolve from the original gene for which the group is named. For example, as of 2014 the blaOXA-23-like group included 19 genes, including including blaOXA-23, -27, -49, and -73. PMID: 24696435. The OXA-48 like class D beta-lactamase enzymes are highly resistant to penicillins and beta lactamase inhibitors and show a low resistance to carbapenems. These varied OXA-48-like beta-lactamases have been identified worldwide, including OXA-48-like producing Klebsiella pneumoniae with resistance to multiple drugs that emerged in Jordanian hospitals. Reports have shown that organisms producing OXA-48 like enzymes are either not resistant to broad-spectrum cephalosporins or show a decreased susceptibility to carbapenemase drugs. Verona integron-encoded metallo-β-lactamase (VIM) is part of a growing family of carbapenemases and includes at least 10 members. VIMs are one of the most common types of carbapenemases, with many variants in at least 23 species of Gram-negative bacilli from more than 40 regions of the world. Mycoplasma pneumoniae is a filamentous cell without a cell well. It has an attachment organelle at its tip with protein adhesions which attach to sialoglycoproteins or sialoglycolipid receptors on the tracheal epithelium. Pneumonia is induced by local immunologic and phagocytic responses.
Mycoplasmas generally are the smallest self-replicating organisms possessing the smallest genomes (500 to 1000 genes). Mycoplasmas have several surface antigens; antibodies to these surface antigens inhibit their growth. While various serological tests have been developed and are useful in classification, the infections are persistent, frequently difficult to detect and difficult to cure. Mycoplasma pneumonaie infection affects the upper and lower respiratory tracts. Symptoms including cough, fever and headache may persist for several weeks. Adenoviruses, medium-sized (90-100 nm), non-enveloped icosohedral viruses with double-stranded DNA, have over 50 immunologically distinct types that cause infections in humans. Adenoviruses usually cause respiratory illness with symptoms ranging from the common cold to pneumonia, croup, and bronchitis. Adenoviruses can cause other illnesses such as gastroenteritis, conjunctivitis, cystitis, and less commonly, neurological disease. Infants and individuals with weakened immune systems are at high risk for severe complications of adenovirus infection and adenoviruses commonly cause acute respiratory illness in military recruits. Some asymptomatic people infected with adenoviruses have ongoing infections in their tonsils, adenoids, and intestines and can shed the virus for months or years. The virus can persist outside the body for long periods. Adenovirus infections can be detected using antigen detection, polymerase chain reaction assay, virus isolation, and serology. Typing is usually done by hemagglutination-inhibition and/or neutralization with type-specific antisera or by molecular methods. As of October 2014 there is no adenovirus vaccine for the general public, however an oral vaccine against types 4 and 7 was approved by the U.S. Food and Drug Administration in March 2011 for U.S. military personnel. The vaccine is recommended by the U.S. Department of Defense for military recruits entering basic training in order to prevent acute respiratory disease. Most adenovirus infections are mild and typically require only treatment of symptoms.

HRVs, which are the cause of more than one-half of cold like illnesses, are commonly associated with upper respiratory tract infection, otitis media, and sinusitis. PCR testing for respiratory virus detection has led to the recognition of HRV as a lower respiratory tract pathogen as well, especially in patients with asthma, infants, elderly patients, and immunocompromised individuals. At this time there are no approved antiviral therapies, and treatment is primarily supportive.

Influenza viruses, from the family Orthomyxoviridae, are enveloped negative-strand RNA viruses with segmented genomes containing seven to eight gene segments. Of the five genera in the family, only influenza A virus (IAV), influenza B and C are medically relevant to humans. Aquatic birds harboring avian IAV are the likely natural reservoir for all known IAV subtypes and are probably the original source of all human pandemic IAV strains. IAV maintained in wild birds have been associated with stable host switch events to novel hosts, including most often to domestic poultry but also to horses, swine, toothed whales and humans. Influenza viruses A and B are single-stranded RNA enveloped viruses belonging to the Orthomyxoviridae family. The symptoms of infection of Influenza A and B include fever, cough, sore throat, rhinorrhea, nasal congestion, headache, myalgia and malaise. Some patients may experience gastrointestinal symptoms including watery diarrhea, vomiting and abdominal pain. The influenza viruses spread rapidly in enclosed spaces with a documented rate of infection of 70% following a common source of exposure. Infection occurs in the respiratory epithelium in both the upper and lower respiratory tract, and the virus can be found in many specimen types, including nasal aspirates, nasal or nasopharyngeal swabs, throat swabs, sputum and tracheal aspirates. Middle East Respiratory Syndrome (MERS) is an illness caused by the virus Middle East Respiratory Syndrome Coronovirus (MERS-CoV). The illness was first reported in Saudi Arabia in 2012, but was later found to have originated in Jordan in 2012. MERS has been found in camels leading to speculation that they may be the initial reservoir for the virus. The largest outbreak of MERS that has occurred outside of the Arabian Peninsula was in the Republic of Korea in 2015 and was linked to an individual who had been traveling in the Arabian Peninsula. The virus is probably spread through respiratory secretions from infected individuals though the exact mode of transmission is under investigation. Infection with MERS begins with an incubation period of 5-6 days followed by a severe acute illness that involves fever, shortness of breath and for patients with comorbidities or reduced immunity can lead to pneumonia and kidney failure. The mortality rate of MERS is 3-4 out of 10 people who are infected, though most of the people who die have underlying conditions. In addition, some people experience only cold-like symptoms and others have no symptoms at all. In the United States, most state laboratories are approved to test for MERS-CoV using the real-time reverse transcription-PCR assay developed by the CDC. Parainfluenza virus is from the family Paramyxoviridae and the genera Paramyxovirus which includes the parainfluenza viruses and the mumps virus. Paramyxoviruses are particles up to 300 nm in diameter. They are spiral, tube-like symmetrical nucleocapsids which contain a monopartite, single-stranded, negative-sense RNA genome and an RNA-directed RNA polymerase. There is a matrix protein at the base of a double-layered lipid envelope which contains spikes with two glycoproteins, a viral attachment protein and a fusion protein. The viral protein spikes in parainfluenza viruses have hemagglutinating and neuraminidase activities. As all paramyxoviruses are labile, they are quickly inactivated (e.g., by heat, detergents, ultraviolet light). Primary infections occur in young children and cause mild or severe upper and lower respiratory tract infections. Reinfection is common and results in milder manifestation.
