Term Description
The Risk Adjustment Factor score (RAF) is calculated by the Centers for Medicare & Medicaid Services (CMS) on a quarterly basis. An individual patient's RAF is based on a combination of Hierarchical Condition Categories (HCCs), which are associated with ICD-10 diagnoses, and patient demographics (including gender and age). RAFs are used to calculate annual expenditure for an individual patient and help providers and insurers determine the potential financial risks of patient care. [www.aafp.org/fpm/2016/0900/p24.html]
