This test measures the valine level in patients who have been diagnosed with maple syrup urine disease are on a therapeutic diet for their condition. Depending on the lab, the valine level may be used in combination with levels of other amino acids such as leucine, isoleucine, and alloisoleucine to monitor the patient's progress.

Valine, one of the 20 proteinogenic amino acids, is nutritionally essential. It is named after the plant valerian. Foods that are good sources of valine include cottage cheese, fish, poultry, peanuts, sesame seeds, and lentils. In sickle cell disease, the replacement of an A by T in the beta chain gene of hemoglobin changes the codon GAG (for glutamic acid) to GTG (which encodes valine). Thus valine, a hydrophic amnio acid, replaces glutamic acid, a hydrophilic amino acid, causing hemoglobin to not fold correctly. It is also a precursor in the penicillin biosynthetic pathway.
