LP263522-7 Clostridioides difficile BI-NAP1-027 strain
Clostridium difficile (CD) infections in hospitals across the United States, Canada and Europe experienced an enormous increase between 2000 and 2006. The pathogen that was found to cause this outbreak was the strain of CD determined by restriction endonuclease analysis (REA) to be toxin type III, restriction endonuclease analysis (REA) group B1; by pulsed-field gel electrophoresis as North American pulsed-field type NAP1; and by polymerase chain reaction (PCR) ribotyping as type 027. The strain is identified as Clostridium difficile BI/NAP1/027 or NAP1. The NAP1 strain of CD produces increased levels of three toxins, the usual toxins A and B, and a binary toxin. A mutation of the toxin A and B suppressor gene tcdC may explain the increased production of toxin. It is also strongly resistant to treatment with fluoroquinolone antibiotics. CD NAP1 is not a new strain of CD. It was isolated between 1984 and 1992 and is identical to the strain isolated is the 2000s, except that the original CD NAP1 strain was not resistant to the new fluoroquinolone drugs of the time, which included gatifloxacin and moxifloxacin. Risk factors for infection with CD NAP1 include advanced age, hospitalization, and exposure to antibiotics, notably fluoroquinolones and cephalosporins. The elderly are especially vulnerable to infection, and have a 30-day mortality rate of more than 5%. PMID: 19457419

Clostridium difficile toxin's A and B are encoded by the tcdA and tcdB genes, respectively. PMID: 20664812 They share similar molecular characteristics with comparable sizes (308 kD for tcdA and 269 for tcdB) and 63% homology in their amino acid sequences. PMID: 9312171 TcdA and tcdB are expressed in response to environmental stresses, such as antibiotics.[ PMID 15831824] Nucleic acid sequences within these genes are targeted to confirm the presence of C. difficile toxins and diagnose the cause of diarrhea, abdominal pain, fever, nausea and other toxin-producing symptoms.
