EDDP (2-ethylidene-1,5-dimethyl1-3,3-diphenylpyrrolidine) is a primary metabolite of methadone, a synthetic opioid used for the treatment of opioid dependence. It is excreted in bile and urine along with other another methadone metabolite, EMPD. Levels of methadone itself are variable and depend on a patient's urine pH value, dose given, and metabolism. EDDP levels are less variable and can be detected 4 to 6 hours after methadone use, therefore, EDDP is preferentially used to check for methadone exposure.

Dihydrocodeine, also called DHC, Drocode or DF-118, is a synthetic opioid analgesic prescribed for postoperative pain, severe dyspnea, or as an antitussive. It was developed in the early 1900s, and is similar in chemical structure, pharmacological activity, and potency as codeine. Although dihydrocodeine does have extremely active metabolites, in the form of dihydromorphine and dihydromorphine-6-glucuronide (100x more potent), these metabolites are produced in such small amount that they do not have clinically important effects.

Hydromorphone is a drug used to relieve moderate to severe pain. It is known by the trade name Dilaudid®. An extended-release version called Palladone® SR was available for a short time in the United States before being voluntarily withdrawn from the market after an FDA advisory released in July 2005. Hydromorphone belongs to a category of drugs known as opioid agonists. It is commonly given to patients who have recently undergone surgery or who have suffered serious injury. Hydromorphone is often sought after by opiate drug abusers as it is one of the most potent of all prescription narcotics, when taken parenterally.

Morphine is an extremely powerful opiate analgesic drug and is the principal active agent in opium. Like other opioids such as heroin, morphine acts directly on the central nervous system (CNS) to relieve pain, in particular by targeting the synapses of the arcuate nucleus. Side effects include impairment of mental performance, euphoria, drowsiness, lethargy, and blurred vision. It also decreases hunger, inhibits the cough reflex, and produces constipation. Morphine is highly addictive when compared to other substances, and tolerance and physical and psychological dependence develop quickly. Patients on morphine often report insomnia, visual hallucinations and nightmares.

Norhydrocodone is a metabolite of hydrocodone, a narcotic analgesic related to codeine, but more potent and more addicting by weight. It is used also as cough suppressant. Drug testing of pain patients provides objective information to health specialists regarding patient compliance, diversion, and concurrent illicit drug use. The interpretation of test results can be difficult however because of complex biotransformations of parent drug to metabolites that are also available commercially and may be abused. Additional testing for normetabolites, such as norhydrocodone, provides conclusive evidence that the parent drug was consumed and is helpful in interpreting the opiate drug source, and also reduces potential false negatives. Oxycodone is in the opioid drug class and is a potent pain reliever. Noroxymorphone is a major secondary metabolite of oxycodone. It is formed after O-demethylation of noroxycodone by CYP2D6 and to a lesser extent by N-demethylation of oxymorphone by Cyp3A4 and 2D6. Noroxymorphone has minimal analgesic effect when oxycodone is administered orally or systemically. Noroxymorphone is a powerful agonist to the mu-opioid receptor when oxycodone is administered intrathecally suggesting that noroxymorphone does not readily cross the blood-brain barrier. Oxycodone is a potent and addictive opioid analgesic medication synthesized from thebaine. Its name is derived from codeine, which has a very similar chemical structure. It is effective orally and is marketed in combination with acetylsalicylic acid (Percodan, Endodan, Roxiprin) or paracetamol (Percocet, Endocet, Roxicet, Tylox) for the relief of pain. More recently, ibuprofen has been added to oxycodone (Combunox). Long term users may develop a tolerance or resistance to the drug. Abusers crush the tablets and injest the powder orally, intranasally, or via intravenous, intramuscular or subcutaneous injections.
