An influenza virus that had never been identified as a cause of infections in people before the 2009 H1N1 (Swine flu) pandemic. Genetic analyses of this virus showed that it originated from animal influenza viruses and was unrelated to the human seasonal H1N1 viruses that had been in general circulation among people since 1977. Antibodies to the seasonal H1N1 virus did not protect against the pandemic H1N1 virus, however a significant percentage of people age 65 and older had some immunity against the pandemic virus. After early outbreaks in North America in April 2009 the new influenza virus spread rapidly around the world. Unlike typical seasonal flu patterns, the new virus caused high levels of summer infections in the northern hemisphere, and then even higher levels of activity during cooler months. The virus also led to patterns of death and illness not normally seen in influenza infections. Most of the deaths occurred among younger people, including those who were otherwise healthy.

The influenza virus A (FLUVA) genome is composed of eight gene segments (PB2, PB1, PA, HA, NP, NA, M, NS1) that code for 11 proteins. Two of the genes, hemagglutinin (HA) and neuraminidase (NA), determine the structure of the two primary surface proteins of influenza viruses. The HA and NA gene sequences are used to determine the Influenza virus subtype. Currently there are 16 subtypes of HA and 9 subtypes of NA known, but only H 1, 2 and 3, and N 1 and 2 are commonly found in humans. Term Description
The quantitative cross/cycle threshold (Ct) value may be reported for real-time PCR-based assays. The value is defined by the number of PCR cycles required for the fluorescent signal of a given product (targeted RNA) to cross the threshold (i.e. exceed background level). In general, the lower the Ct value, the greater the amount of targeted nucleic acid and vice versa. The interpretation (e.g. positive, negative, indeterminate) is determined based on a cut-off value. The submitter's lab reports the interpretation in OBX.8 segment and the specimen type in SPM.4 field of HL7 messaging.

Term Description
The goose/Guangdong lineage H5 viruses emerged in the late 90s in Asia and subsequently became the only lineage of viruses known to be highly pathogenic to poultry and sustained in wild birds. The World Health Organization recognizes these viruses under a unified H5 clade nomenclature. Viruses from this lineage, specifically clade 2.3.4.4, were introduced into North American flyways in late 2014 via the Bering Strait into the Pacific flyway and again in late 2021 into the Atlantic flyway. PMID: 25855748 The NALHN influenza A molecular assay (matrix 2009) works well for the detection of this viral clade and is the primary surveillance tool. As a screening tool, the NAHLN H5 assay does not distinguish geographic lineage nor pathotype (LPAI vs HPAI). This clade specific assay uses real-time reverse transcriptase PCR targeted to clade H5 2.3.4.4.

LP208510-0 Influenza virus A H7 Eurasia
Human infections with a new avian influenza A (H7N9) virus were first reported in China in March 2013. Most of these infections are believed to result from exposure to infected poultry or contaminated environments, as H7N9 viruses have also been found in poultry in China. While some mild illnesses in human H7N9 cases have been seen, most patients have had severe respiratory illness, with about one-third resulting in death. Rare, limited person-to-person spread of this virus has been identified in China, but there is no evidence of sustained person-to-person spread of H7N9. The first case outside of China was in Malaysia and was reported on February 12, 2014. The case was detected in a traveler from an H7N9-affected area of China. The new H7N9 virus has not been detected in people or birds in the United States as of this writing.

The value is defined by the number of PCR cycles required for the fluorescent signal of a given product (targeted RNA) to cross the threshold (i.e. exceed background level). In general, the lower the Ct value, the greater the amount of targeted nucleic acid and vice versa. The interpretation (e.g. positive, negative, indeterminate) is determined based on a cut-off value. The submitter's lab reports the interpretation in OBX.8 segment and the specimen type in SPM.4 field of HL7 messaging.

Influenza viruses, from the family Orthomyxoviridae, are enveloped negative-strand RNA viruses with segmented genomes containing seven to eight gene segments. Of the five genera in the family, only influenza A virus (IAV), influenza B and C are medically relevant to humans. Aquatic birds harboring avian IAV are the likely natural reservoir for all known IAV subtypes and are probably the original source of all human pandemic IAV strains. IAV maintained in wild birds have been associated with stable host switch events to novel hosts, including most often to domestic poultry but also to horses, swine, toothed whales and humans. Influenza virus from the family Orthomyxoviridae are enveloped negative-strand RNA viruses with segmented genomes containing seven to eight gene segments. Of the five genera in the family, only influenza A virus (IAV), influenza B and C viruses are medically relevant to humans. Influenza A and B viruses have a similar structure whereby these viruses each contain eight discrete single-stranded RNA gene segments, each encoding at least one protein. While having a close phylogenetic relationship, influenza virus types A and B manifest very differently. While IAV infects birds, humans and other mammals, influenza B only infects humans and seals. Although both viruses cause human respiratory infection seasonally in temperate climate regions and more continually in the tropics, influenza B is normally less prevalent and causes less severe disease than IAV viruses. Term Description
For the qualitative assay of influenza virus type B viral RNA. In the upper respiratory tract clinical specimens to include: nasopharyngeal swabs, nasal swabs, throat swabs, nasal aspirates, nasal washes and dual nasopharyngeal/throat swabs. Possible lower respiratory tract specimens include bronchoalveolar lavage, bronchial washings, tracheal aspirate, sputum, and lung tissue from patients with signs and symptoms of respiratory infection. This assay identifies the genetic lineage of human influenza B viruses as B/Victoria. This term was developed for, but not limited in use to, the CDC Human Influenza Virus Real-Time RT-PCR Diagnostic Panel.
