Cholestanol (C27H47OH) is formed from the reduction of cholesterol and a marker used to evaluate cholesterol absorption and synthesis disorders, including Smith-Lemli-Optiz syndrome (SLOS) and cerebrotendinous xanthomatosis (CTX). Patients with CTX have elevated cholestanol levels (3-15 times higher, ranging from 1.3-15 mg/d) and low-to-normal cholesterol levels (usually 115-220 mg/dL). For CTX, the cholesterol-to-cholestanol ratio is said to be a better indicator of disease than cholestanol concentration alone. Source: Regenstrief LOINC
