Description

A medication administration flowsheet, commonly known as a medication administration record (MAR) or drug chart, is a summary of all of the medications administered to a patient over a certain period of time, such as during the course of a hospital admission or periodic home nursing visits. The flowsheet typically includes information about the patient (name, date of birth, medical record number, gender, allergies, diagnosis, special instructions, etc.), the prescribing doctor, and for each medication, details about the prescription as well as the administration. Prescription data may include drug name, drug form (e.g., tablet, injection), dose, route, frequency, and scheduled time of administration; administration data include the actual time of administration, the dose given, the site (for example, G-tube, IV left hand, or right deltoid), or other relevant information such as a missed dose or emesis post-administration.
