Description

The compounds known as novel synthetic opioids (NSOs) have been emerging as drugs of abuse, given that many share pharmacological properties with common opioid agonists, yet they are difficult to detect and control. Since the early 2010s clandestine manufacturing of both novel fentanyl derivatives as well as other synthetic opioids has increased, with the list growing to upwards of more than 30 unique compounds. The list includes fentanyl analogs such as acetylfentanyl, butyrfentanyl, 4-methoxybutyrylfentanyl, 4-methylphenethylacetylfentanyl, furanylfentanyl, carfentanil (and its major metabolite norcarfentanil), ocfentanil, acrylfentanyl, 3-methylfentanyl and 4-fluorobutyrylfentanyl. Also, in the same category are non-fentanyl derived chemicals having opioid agonist activity such as AH-7921 and U47700(benzoamides) and MT-45 (a piperazine). The pharmacologic receptor profile of some predict that they would be highly addictive and in some cases much more potent (for example, 3-Methylfentanyl is about 1000 times and carfentanil may be 10,000 times more potent than morphine). The fact that they are often cut into heroin preparations or sold as counterfeit pain killers with users unaware of their content makes them particularly prone to overdose.
